Seen something the register has not

Send the link. The pipeline reads the article, looks for a second independent source, dates the event, names the machine and strips private names, exactly as it does for every find of its own. Nothing is published on a single source, and a submission is never published as sent.

What counts: a commercial robot, robotaxi, delivery robot, humanoid, warehouse, surgical or service machine that hurt someone, damaged something, failed in public, was recalled, investigated, fined or sued. What does not: concept demos, research prototypes, military systems, and software with no machine. The rules are on the methodology page. To correct a case already on the register, use corrections; a company named in a case has a right of reply.

A news article, a regulator notice, a court docket or a company statement. Not a social post on its own: those cannot be verified.

Optional. Where, when, which machine. No names of private individuals, please.

Optional. Used once, to tell you if the case is published. Never shown, never shared.
