A Zoox vehicle operating in autonomous mode was traveling west in the left through lane on [XXX], past [XXX], in Las Vegas when a passenger car (Vehicle 2) traveling ahead in the adjacent lane abruptly swerved hard left, in an apparent attempt to make a sudden U-turn, across the path of the approaching Zoox vehicle. The Zoox vehicle braked hard; just as its operator then disengaged from autonomy, steered left, and continued to brake, but the right front corner of the Zoox vehicle made contact with the left side of Vehicle 2. Police were notified and arrived at the scene. Both vehicles sustained damage, and the Zoox vehicle was towed from the scene. No injuries were reported.

On September [XXX], 2026, at approximately 11:50 PM PT, a Motional IONIQ 5 Robotaxi was operating in autonomous mode in Las Vegas, Nevada, with a Safety Driver in the driver’s seat. The Motional vehicle was traveling northbound on [XXX] in the rightmost lane, approaching [XXX]. The Motional vehicle planned to take a right turn on [XXX] and activated its right-turn signal 13 seconds before the turn. At that time, an individual operating a motorized e-bike approached from behind the Motional vehicle and transitioned from the bike lane into the travel lane directly behind the Motional vehicle. While the Motional vehicle slowed in preparation for the turn, with its right-turn signal still active, the e-bike operator moved toward the right side of the lane and attempted to pass the turning Motional vehicle on its right. The left side of the e-bike contacted the right side of the Motional vehicle after the vehicle had begun the right turn. The Safety Driver disengaged the autonomous system immediately following the collision. The e-bike operator stood and walked from the roadway unassisted. The e-bike operator declined medical treatment at the scene and no airbags deployed. The Motional vehicle sustained minor cosmetic damage, remained drivable, and was not towed. Officers from the [XXX] responded to the scene and will prepare a report.

On August [XXX], 2026 at 11:17 PM PDT a Waymo Autonomous Vehicle ("Waymo AV") operating in Las Vegas, Nevada was in a collision involving a passenger car on [XXX] near the [XXX] overpass. The Waymo AV was traveling northbound in a queue of traffic in the rightmost lane on [XXX], when a passenger car traveling northbound directly in front of the Waymo AV made contact with the rear of an SUV stopped in-lane in the queue of traffic. The Waymo AV slowed to a stop behind the passenger car and the passenger car reversed, and the rear of the passenger car made contact with the front of the stationary Waymo AV. At the time of the impact, the Waymo AV's Level 4 ADS was engaged in autonomous mode, and a test driver was present in the driver's seating position. The Waymo AV sustained damage. Waymo is reporting this crash under Request No. 2 of Standing General Order 2021-01. Waymo may supplement or correct its reporting with additional information as it may become available.

A Zoox vehicle in autonomous mode was traveling north on [XXX] toward [XXX] in Las Vegas, slowing gradually for a slowing tractor trailer (Vehicle 2) in its travel lane ahead. Vehicle 2 then began reversing toward the nearly stopped Zoox vehicle. The Zoox vehicle operator disengaged from autonomy and the rear of Vehicle 2 made contact with the front of the Zoox vehicle. Following the collision, the Zoox vehicle operator turned right onto Convention Center Drive, pulled over, and the vehicle was towed shortly after. No injuries were reported and police were not notified.

An unoccupied Zoox autonomous vehicle was northbound on [XXX] approaching the intersection of [XXX] in Las Vegas when an active emergency vehicle, with lights and sirens, was eastbound in the right through lane on [XXX] and approaching the intersection. The Zoox vehicle slowed as it entered the intersection and braked hard to a stop in the intersection to yield to the approaching emergency vehicle. An SUV (Vehicle 2) traveling behind the Zoox vehicle failed to stop and rear ended the stopped Zoox vehicle. Following the collision, the emergency vehicle changed course and went around the two vehicles in the intersection. The police were notified and arrived at the scene. Both vehicles were towed from the scene.

On August [XXX], 2026, at approximately 2:00 PM PT, a Motional Ioniq 5 Robotaxi (VIN [XXX]) was operating in autonomous mode in Las Vegas, Nevada, with a Vehicle Operator in the driver’s seat. The Motional vehicle was traveling eastbound on [XXX], between [XXX] and [XXX]. The vehicle traveling directly ahead of the Motional vehicle decelerated rapidly. In an abundance of caution, the Vehicle Operator disengaged the autonomous system and applied the brakes in manual mode to avoid a collision with the vehicle ahead. After the Motional vehicle decelerated, a white BMW SUV traveling behind the Motional vehicle struck the rear of the Motional vehicle. Following the collision, the Vehicle Operator reported minor injuries and was evaluated by medical personnel, who cleared the Vehicle Operator to return to work that day. No airbags deployed. The Motional vehicle sustained damage to its rear but remained drivable and was not towed from the scene. A collision report was filed with the Las Vegas Metropolitan Police Department.

An occupied Zoox autonomous vehicle was traveling north in the left through-lane of Las Vegas Boulevard, past [XXX], in Las Vegas. An SUV (Vehicle 2) traveling slightly ahead in the adjacent right lane executed an abrupt lane change into the Zoox vehicle's lane, then immediately braked hard to a stop to avoid a jaywalking pedestrian who was moving east-to-west across the northbound lanes. In response, the Zoox vehicle braked hard and steered left; however, the right front corner of the Zoox vehicle made contact with the left rear corner of Vehicle 2 before coming to a stop. Following the collision, both vehicles moved over to the left turn-lane to exchange information. Law enforcement responded to the scene and no injuries were reported.The Zoox vehicle was subsequently towed for inspection; inspection confirmed it sustained minor damage.

On August [XXX], 2026, at approximately 12:40 PM PT, a Motional Ioniq 5 Robotaxi (VIN [XXX]) was operating in autonomous mode in Las Vegas, Nevada, with a Vehicle Operator in the driver’s seat. The Motional vehicle was traveling within the[XXX] shopping mall complex. While executing a left-turn maneuver onto [XXX], the Motional vehicle encountered a [XXX] delivery truck parked with its liftgate in the lowered position. The [XXX] truck was blocking the vehicle’s intended path through the intersection, and the Motional vehicle initiated an S-curve maneuver to route around the parallel parked truck.. While completing the S-curve maneuver, the Motional vehicle contacted the curb and made physical contact with a small palm tree approximately three feet tall located near the curb. The Vehicle Operator disengaged the autonomous system at the time of impact and assumed manual control to maneuver the vehicle to a safe location. No injuries were reported, and no airbags deployed. The Motional vehicle was not towed from the scene and remained drivable.

An unoccupied Zoox autonomous vehicle was stopped in the outer left-turn lane on westbound [XXX], preparing to turn left onto South [XXX] in Las Vegas. Directly behind it, a pickup truck (Vehicle 2) failed to maintain a safe following distance and intentionally approached extremely close to the stationary Zoox vehicle just prior to the traffic signal turning green. As the light changed to green, the Zoox vehicle began moving but subsequently slowed at which point it was struck by Vehicle 2. The driver of Vehicle 2 then reversed and maneuvered around the stopped Zoox vehicle before leaving the scene without exchanging information. The Zoox vehicle was found to have sustained minor damage. No injuries were reported and police were not notified.

On July [XXX], 2026 at 9:40 AM PDT, a Waymo Autonomous Vehicle ("Waymo AV") operating in Las Vegas, Nevada, was in a collision involving an SUV on [XXX] near [XXX]. The Waymo AV was traveling southbound on [XXX] with its right turn signal activated and slowing to yield for a stop sign at the intersection with [XXX]. As the Waymo AV was slowing, an SUV traveling southbound on [XXX] began to maneuver to the right as it approached the Waymo AV from the rear and the front left side of the SUV made contact with the rear right side of the Waymo AV. The SUV then proceeded to the right and the front of the SUV made contact with a utility pole on the west curb of [XXX], at which point the SUV traveled backwards towards the Waymo AV, and the rear left side of the SUV made secondary contact with the right side of the stationary Waymo AV. At the time of the impacts, the Waymo AV's Level 4 ADS was engaged in autonomous mode. Both vehicles sustained damage. The driver of the SUV was transported from the scene to a hospital for medical treatment with serious injuries, and the passenger of the SUV was transported from the scene to a hospital for medical treatment with unknown injuries. Waymo is reporting this crash under Request No. 1 of Standing General Order 2021-01 because an individual involved was transported from the scene to a hospital for medical treatment. Waymo may supplement or correct its reporting with additional information as it may become available.

On July [XXX], 2026, at approximately 7:32 PM PT, a Motional Ioniq 5 Robotaxi with VIN [XXX] was operating in autonomous mode in Las Vegas, Nevada, with a Safety Driver in the driver’s seat. The Motional vehicle was traveling in the roadway area near the [XXX] shopping mall and proceeding south on a parking lot street. The Motional vehicle encountered a parked semi-truck and trailer that was fully blocking its lane of travel. The Motional vehicle initiated a maneuver to safely navigate around the parked semi-truck by momentarily entering the opposite lane of travel before intending to return to the south bound lane. The Motional Vehicle initiated the maneuver and when reaching the point at which it was parallel with the parked semi-truck trailer,the Vehicle Operator disengaged the autonomous system and assumed manual control to complete the maneuver. The Motional Vehicle returned to the Motional facility at the end of operation as planned. Upon a routine inspection of the vehicle, it was discovered by facilities staff that the vehicle had sustained minor cosmetic damage which does not require repair. No injuries were reported, and no airbags deployed. The Motional vehicle was not towed from the scene. Law enforcement was not contacted, and no police report was filed.

An unoccupied Zoox autonomous vehicle was traveling south in the right lane on [XXX] toward [XXX] in Las Vegas, when it began slowing for emergency personnel in an active, unblocked emergency fire scene, with heavy smoke obscuring the road ahead. The Zoox vehicle entered the emergency scene, and just before it slowed to a stop, one of the first responders made intentional contact with the front of the vehicle with a hand tool. The Zoox vehicle then returned to base for inspection and was found to have sustained moderate damage from the intentional contact. No injuries were reported.

On June [XXX], 2026, at approximately 2:27 PM PT, a Motional Ioniq 5 Robotaxi (VIN [XXX]) was operating in autonomous mode in Las Vegas, Nevada, with a Vehicle Operator in the driver’s seat. The Motional vehicle was traveling southbound on [XXX], south of [XXX], in the rightmost of two lanes. A bus was stopped in the Motional vehicle's lane of travel directly ahead. During the lane-change maneuver to move around the bus, the AV signaled and began positioning for a left-lane change but stopped due to traffic. In order to complete the lane change, the Vehicle Operator disengaged the AV system and manually moved the Motional vehicle toward the left lane, where a rapidly approaching vehicle clipped the Motional vehicle's front left quarter panel while passing. The other vehicle did not stop following the collision and fled the scene. Following the impact, the Vehicle Operator pulled over to the side of the roadway. No injuries were reported, and no airbags deployed. The Motional vehicle sustained damage to its front left quarter panel but remained drivable. The Motional vehicle had been in manual control for less than 30 seconds before impact. Law enforcement was contacted, and a police report was filed.

A Zoox vehicle, operating in autonomous mode, was heading south on [XXX] in Las Vegas, slowing down for a red traffic signal at the [XXX] intersection, when a trailing passenger car (Vehicle 2) rear-ended it. The Zoox vehicle operator subsequently disengaged from autonomy. The collision resulted in moderate rear-end damage to the Zoox vehicle and major frontal damage to Vehicle 2, including frontal airbag deployment. Both vehicles were towed from the scene. Police responded to the scene, and no injuries were reported.

An unoccupied Zoox autonomous vehicle was making a left turn from eastbound [XXX] onto northbound [XXX] in Las Vegas when an active emergency vehicle, with lights and sirens, was rapidly approaching the intersection and conducting a right turn onto northbound [XXX]. The Zoox vehicle was slowing to yield to the approaching active emergency vehicle, when an SUV (Vehicle 2) traveling behind it failed to stop, and rear ended the slowing Zoox vehicle. Two other passenger vehicles (Vehicles 3 & 4) were following behind Vehicle 2. Vehicle 4 appeared to have made contact with Vehicle 3 from behind. Neither Vehicle 3 nor Vehicle 4 contacted the Zoox vehicle, and it does not appear that either contacted Vehicle 2. The driver of Vehicle 2 then pulled over ahead and contacted Zoox support to exchange information. The police were notified, but did not arrive at the scene. The Zoox vehicle was towed from the scene and found to have sustained moderate damage.

An occupied Zoox autonomous vehicle was traveling north on [XXX], approaching the intersection of [XXX] in Las Vegas, when an active emergency vehicle, with lights and sirens, was westbound on [XXX] and rapidly approaching the intersection. The Zoox vehicle appropriately braked hard to a stop to yield to the approaching active emergency vehicle. A pickup truck (Vehicle 2), traveling behind the Zoox vehicle, failed to stop and rear ended the stopped Zoox vehicle. The police were notified and arrived at the scene. Zoox vehicle's camera footage shows that the driver of Vehicle 2 was holding a device in their right hand leading up to the collision. The driver of Vehicle 2 was cited for failing to decrease speed. The Zoox vehicle was towed from the scene and found to have sustained moderate damage. The passengers initially declined medical attention; however, after the Zoox fleet response team arrived, one passenger requested transport to a hospital.

An occupied Zoox autonomous vehicle was traveling north on [XXX] in the left through lane, approaching [XXX] in Las Vegas, when a passenger vehicle (Vehicle 2) in the outer left-turn lane (towards Aria Place) abruptly changed lanes to the right and cut in front of the Zoox vehicle. The Zoox vehicle braked and attempted to steer right to avoid the collision, but the right side of Vehicle 2 swiped the left side door and body panels, and fenders of the Zoox vehicle. The driver of Vehicle 2 departed the scene without exchanging information. The Zoox vehicle was towed from the scene and was found to have sustained moderate damage. There were no reported injuries.

On May [XXX], 2026, at approximately 8:05 AM PT, a Motional Ioniq 5 Robotaxi (VIN [XXX]) was operating in autonomous mode in Las Vegas, Nevada, with a Vehicle Operator in the driver's seat. The Motional vehicle was traveling eastbound on [XXX] near the intersection with [XXX] and came to a stop at a red traffic signal. While stopped, the Vehicle Operator disengaged autonomous mode and assumed manual control to perform a lane change to travel southbound on[XXX]. As the Vehicle Operator maneuvered the Motional vehicle into the right-turn lane in manual mode at approximately 2 mph, the passenger-side front bumper and fender of the Motional vehicle struck another vehicle that was occupying the lane. The autonomous system had been disengaged less than 30 seconds before the impact.

An unoccupied Zoox autonomous vehicle (Vehicle 1) was traveling north on [XXX], having just completed a left turn from [XXX] in Las Vegas, when it encountered debris in its path. The Zoox vehicle attempted to steer right, then executed hard braking to avoid the debris, at which point it was rear-ended by a passenger car (Vehicle 2) traveling directly behind it. Both vehicles came to a stop following the impact and were subsequently towed due to the damage sustained. Vehicle 2 suffered front-end damage and its driver-side frontal airbags deployed, while the Zoox vehicle sustained moderate damage to its rear. Law enforcement was notified of the event, but did not respond to the scene. Following the impact, Zoox staff inquired if emergency medical services should be called; the driver replied that the police had already been contacted. The driver of Vehicle 2 informed Zoox staff at the scene that they were "shaken" and "pretty bruised".

An unoccupied Zoox autonomous vehicle was in the left lane traveling north on [XXX] past [XXX] when it slowed for a tractor-trailer (vehicle 2) that was reversing from the roadway into a loading bay on the right side of the road. While reversing, the tractor-trailer swung over into the left lane, and the left side of Vehicle 2's trailer made contact with the stopped Zoox vehicle's front fascia and sensor pods. The Zoox vehicle was towed from the scene. No injuries were reported, and police were not notified.

An occupied Zoox autonomous vehicle, gradually slowed to a stop behind traffic on southbound [XXX] past [XXX] in Las Vegas, when it was rear ended by a passenger vehicle (Vehicle 2) traveling directly behind. The Zoox vehicle honked at the approaching Vehicle 2, but its driver failed to stop in time. Vehicle 2's driver left the scene without exchanging information. The Zoox vehicle did not sustain any damage. No injuries were reported at that time. Police were notified. One of the passengers later contacted Zoox support and stated, "I began experiencing significant neck pain consistent with a possible soft tissue injury." The passenger has not provided any documentation to Zoox regarding their alleged injury.

An unoccupied Zoox autonomous vehicle was preparing to turn right from a hotel driveway onto southbound [XXX] in Las Vegas, and came to a gradual stop to yield to cross traffic when it was rear ended by a pickup (Vehicle 2) traveling directly behind. The Zoox vehicle honked at the approaching Vehicle 2, but Vehicle 2's driver failed to stop in time. Vehicle 2's driver then went around the stopped Zoox vehicle, briefly stopped in the right lane, activated their hazard lights, and then proceeded to leave the area. Upon inspection, the Zoox vehicle was found to have sustained moderate damage to its rear. No injuries were reported and police were not notified.

An unoccupied Zoox autonomous vehicle was traveling north on [XXX] past [XXX] in Las Vegas when a passenger car (Vehicle 2) abruptly attempted to turn right onto [XXX] from a parking lot, violating the Zoox vehicle's right-of-way. The Zoox vehicle braked hard and attempted to steer left, but contact was unavoidable. The Zoox vehicle's right-front fender, wheel, and fascia contacted Vehicle 2's left-front bumper and headlight. Both vehicles sustained damage, and the Zoox vehicle was towed from the scene. No injuries were reported, and police were not notified.

On April [XXX], 2026, at approximately 4:42 PM local time, a Motional autonomous vehicle [XXX] was traveling southbound on [XXX] approaching [XXX] in Las Vegas, Nevada. While proceeding within its lane of travel, the vehicle encountered a piece of concrete debris located in the roadway. The vehicle subsequently struck the debris, resulting in a punctured front tire and damage to the vehicle's undercarriage. Following the impact, the safety driver brought the vehicle to a controlled stop and pulled over to the side of the roadway. Due to the damage to the tire, the vehicle was not drivable. A tow truck was requested and the vehicle was subsequently removed from the scene.

An occupied Zoox autonomous vehicle was traveling east at low speed in a parking lot along [XXX] in Las Vegas when an SUV (Vehicle 2) began reversing out of its parking spot. The Zoox vehicle slowed and attempted to steer left, but it could not before Vehicle 2 struck the Zoox vehicle's right-side rear door, body panel, and wheel. The Zoox vehicle sustained moderate damage and was towed from the scene. No injuries were reported, and police were not notified.

An unoccupied Zoox autonomous vehicle was stopped at a LasVegas PUDO location when an SUV (Vehicle 2), that was stopped behind for passenger pick up, attempted to pass on the left and lightly swiped the left rear corner of the Zoox vehicle. The Vehicle 2 driver did not stop at the scene to exchange information. The Zoox vehicle resumed operation to return to base for inspection, where it was found to have sustained minor damage. There were no reported injuries.

An unoccupied Zoox autonomous vehicle started to move after a gradual stop at a 3-way stop sign preparing to turn right onto eastbound [XXX] in Las Vegas, when it braked to an abrupt stop as cross-traffic failed to stop and yield the right-of-way. It was then rear ended by an SUV (Vehicle 2) traveling directly behind. The Zoox vehicle was towed from the scene and was found to have sustained moderate damage to the rear. No injuries were reported and police were not notified. Camera footage from the Zoox vehicle shows that the Vehicle 2 driver was not looking ahead at the roadway before the time of contact.

An unoccupied Zoox autonomous vehicle was traveling northeast on [XXX], approaching [XXX] in Las Vegas, when a passenger car (Vehicle 2) abruptly turned right onto [XXX] from a parking lot on the right, violating the Zoox vehicle's right-of-way. The Zoox vehicle braked hard and attempted to steer left, but contact was unavoidable. The Zoox vehicle's right front fender and wheel, and its right rear door panel, contacted Vehicle 2's left fender, wheel, headlight, and quarter panel. Both vehicles sustained damage, and the Zoox vehicle was towed from the scene. No injuries were reported, and police were not notified.

An occupied Zoox autonomous vehicle traveling southeast on [XXX] stopped behind traffic in the outer left turn lane (lane #2) for a red traffic signal at [XXX] in Las Vegas. A dumpster-hauling truck (Vehicle 2) was stopped in the adjacent through-lane (lane #3). Shortly after the light turned green and the Zoox vehicle began its left turn, the Vehicle 2 operator abruptly steered left, accelerated, and cut into the Zoox vehicle's path, violating its right of way. The Zoox vehicle braked hard; however, Vehicle 2 struck the Zoox vehicle's right-side front wheel, fender, roof-mounted sensor unit, and body panel. The Zoox vehicle was towed from the scene and sustained moderate damage. No injuries were reported, and police were not called.

An occupied Zoox autonomous vehicle traveling south on [XXX] was stopped for a red signal at [XXX] in Las Vegas. After the light turned green, and before the Zoox vehicle started to move, a Pickup (Vehicle 2) traveling directly behind it rear-ended it. Shortly thereafter, both vehicles traveled a few feet ahead and pulled over. The passengers exited the Zoox vehicle onto the sidewalk. After exchanging information, the Zoox vehicle returned to base for inspection, where it was found to have sustained moderate damage. No injuries were reported, and police were not notified.

On Feb [XXX], at approximately 3:16 AM local time, a Motional IONIQ 5 automated vehicle was operating in autonomous mode southbound on Las Vegas Boulevard, traveling at approximately 25 mph. While proceeding in its lane of travel, an individual located near the roadway threw an object toward the front of the vehicle. The object made contact with the lower front bumper area. The vehicle remained under autonomous control and came to a controlled stop following the impact. There were no reported injuries to any occupants or members of the public. No airbags deployed. The vehicle sustained minor cosmetic damage to the lower front bumper and front license plate area. The vehicle was not towed from the scene.

On Wednesday, February [XXX], a vehicle struck the front passenger side of a Motional Ioniq 5 AV in Las Vegas, Nevada. The Motional vehicle was operating in autonomous mode near the intersection of [XXX] and [XXX] when it encountered another vehicle that had stopped in the AV's lane of travel. The AV maneuvered around the stopped vehicle and continued forward. Shortly thereafter, the other vehicle accelerated and attempted to change lanes into the AV's lane from an adjacent lane. The Vehicle Operator disengaged autonomous mode and attempted to avoid the vehicle, but the other vehicle struck the AV's front passenger side. No injuries were reported and the Motional vehicle remained moveable following the collision. The other vehicle did not stop and fled the scene.

On February [XXX], 2026, at approximately 9:55 AM PT, a Motional Ioniq 5 AV (VIN [XXX]) was operating in autonomous mode in Las Vegas, Nevada, with a safety driver in the driver’s seat. The AV was stopped southbound on [XXX] in the left-turn lane at the intersection with [XXX], positioned behind two other stopped vehicles. At that time, a sedan traveling eastbound on [XXX] was being actively pursued by a law enforcement vehicle which had its flashing lights and siren activated. Both vehicles turned left and proceeded northbound on [XXX] but using the southbound lanes. As the sedan traveled in the wrong lanes on [XXX], it struck the passenger-side front end and right wheel well of the Motional vehicle which remained stopped in the left turn lane the entire time. The sedan did not stop after striking the Motional vehicle and continued traveling in the wrong (southbound) lanes and counter to the flow of traffic while it continued fleeing from police. No occupants of the Motional vehicle were injured. The Motional vehicle sustained damage to the front end and was towed from the scene. The Las Vegas Metropolitan Police Department is investigating the incident, but a police report is not yet available

An occupied Zoox autonomous vehicle was traveling northeast on [XXX], passing through the intersection of [XXX] in Las Vegas, with a green signal, when an SUV (Vehicle 2) violated the Zoox vehicle's right of way and abruptly entered its travel path. The Zoox vehicle braked hard and attempted to veer left, but the right front fender of the Zoox vehicle made contact with the left front fender and bumper of Vehicle 2. The driver of Vehicle 2 briefly stopped and then drove away from the scene. The Zoox vehicle was found to have sustained minor damage. At the time of the incident, the passengers did not report any injuries, and police were not notified. On March 2, 2026, the passengers notified Zoox Support that they "sustained multiple injuries and have experienced persistent and worsening pain and cognitive symptoms since the impact".

An occupied Zoox autonomous vehicle (Vehicle 1) was stopped behind traffic in the left turn lane on southbound [XXX] at [XXX] in Las Vegas, waiting at a traffic signal. After the light turned green, the Zoox vehicle initially remained stationary and then started moving forward. Shortly after resuming motion, the Zoox vehicle stopped and was rear-ended by the SUV (Vehicle 2) directly behind it. The Zoox vehicle sustained minor damage, and no injuries were reported. The police were not called.

A Zoox vehicle (Vehicle 1) in autonomous mode was traveling east towards the Passenger Pick-up area in Terminal 3 of [XXX] in Las Vegas. An SUV (Vehicle 2), traveling behind in the left adjacent lane, abruptly attempted to overtake and cut in front of the Zoox vehicle in an apparent attempt to change lanes to the right towards Terminal 3 Departures. The Zoox vehicle braked hard in response to the encroaching vehicle, and the operator then disengaged from autonomy and continued to brake, but contact could not be avoided. The right rear side of Vehicle 2 swiped the left front area of the Zoox vehicle. Both operators stopped after the collision and exchanged information. Both vehicles sustained moderate damage, and the Zoox vehicle was subsequently towed from the scene. There were no reported injuries, and police were not called.

An occupied Zoox autonomous vehicle, southbound on [XXX], was gradually slowing and yielding to cross traffic in preparation for making a right turn at a traffic signal onto [XXX] in Las Vegas, when it was rear ended by a pickup (Vehicle 2) traveling directly behind. The Zoox vehicle came to a stop after the collision. The driver of Vehicle 2 initially exited their vehicle to check the vehicle, then completed the turn, stopped, and then exited their vehicle again to inspect for damage, signaling a "thumbs-up" to the Zoox rider. Vehicle 2 driver did not remain on scene to exchange information. The Zoox vehicle resumed operation and returned to base, where it was found to have sustained minor damage. There were no reported injuries. Camera footage from the Zoox vehicle shows that the Vehicle 2 driver was not looking ahead at the roadway before the time of contact.

On October [XXX], 2025, at approximately 2:30 PM PT, a Motional AV with VIN [XXX] was operating in autonomous mode in Las Vegas, Nevada, with a safety driver in the drivers seat. The vehicle was traveling northbound on [XXX] in the second of four lanes from the left, approaching the intersection with [XXX]. While stopped at a red traffic signal, an adult male pedestrian began crossing [XXX] from the west side of the street, several hundred feet away from a designated crosswalk. The pedestrian proceeded southbound across multiple lanes and paused on a median located ahead and to the left of the AV. The AV detected and classified the pedestrian the entire time. When the traffic signal turned green, the pedestrian remained stopped on the median. Given that the pedestrian remained stopped in place, the AV began to move forward slowly at approximately 2.5 mph and continued to correctly detect and classify the pedestrian. The pedestrian, holding what appears to be an item in a plastic bag, suddenly stepped into the far-left turn lane ahead of the AV, the AV immediately came to a complete stop still in the second lane. The pedestrian continued crossing, making a light physical contact with the rear driver’s side of the vehicle before continuing across the remaining three lanes of traffic still outside of the crosswalk. The vehicle operator then disengaged the autonomous system and pulled over to complete an incident report. No police were notified. There were no injuries, no damage, and no airbag deployments. The pedestrian did not stop at the scene and the identity of the pedestrian is unknown.

On October [XXX], 2025, at 2:37 PM PT, a Motional AV with VIN [XXX] was operating in autonomous mode in Las Vegas, Nevada, with a safety driver in the driver's seat. The vehicle was traveling northbound on [REDACTED, MAY CONTAIN CONFIDENTIAL BUSINESS INFORMATION] in the rightmost of two northbound lanes. The AV was preparing to make a right turn onto [REDACTED, MAY CONTAIN CONFIDENTIAL BUSINESS INFORMATION]. As a pedestrian was in the crosswalk, the AV slowed to a stop. The vehicle operator then disengaged the autonomous system and assumed manual control. While the Motional vehicle was stopped, a passenger vehicle traveling behind attempted to overtake. In moving into the left lane, the passenger vehicle's maneuver resulted in a collision with the rear of the Motional vehicle. An unoccupied Zoox autonomous vehicle was traveling in the right lane on southbound [XXX] towards [XXX] in Las Vegas. The driver of an SUV (Vehicle 2) traveling ahead in the same direction in the adjacent lane to the left suddenly veered right into the path of the Zoox vehicle without using a turn signal, making contact with the left rear wheel and fender, and left side body panels, doors, and rocker. The Zoox vehicle then gradually braked to a stop in the right lane. The Zoox vehicle was towed from the scene for inspection. The Zoox vehicle sustained moderate damage, while Vehicle 2 appears to have sustained damage to the right side fender panel and front bumper. There were no reported injuries, and the police were not called. Zoox is submitting this updated report to correct a previous reporting error in the location field.

As the Zoox vehicle gradually slowed to a stop, the operator of Vehicle 2 began to reverse. The Zoox vehicle honked at Vehicle 2, but Vehicle 2 continued to reverse and made contact at low speed with the right front fender and bumper of the Zoox vehicle. The Zoox vehicle was towed from the scene for inspection and was found to have sustained minor damage. There were no reported injuries, and the police were not called.

An unoccupied Zoox autonomous vehicle was operating at a reduced speed in lane #5 on westbound [XXX] towards Las Vegas Boulevard in Las Vegas, with a sedan (Vehicle 2) directly behind. The Zoox vehicle gradually slowed to a stop when it was rear ended by Vehicle 2. Vehicle 2 driver initially exited his vehicle to inspect both vehicles, but did not remain at the scene to exchange information. The Zoox vehicle resumed operation and cleared the scene. Shortly afterward, the Zoox vehicle slowed to a stop due to a low tire pressure fault and was subsequently towed. The low tire pressure was attributed to the crash. The Zoox vehicle sustained minor damage. There were no reported injuries, and the police were not notified.

On September [XXX], 2025, at approximately 3:30 PM PT, a Motional AV with VIN [XXX] was operating in autonomous mode in Las Vegas, Nevada. A safety driver was in the driver's seat. The AV was preparing to turn [REDACTED, MAY CONTAIN CONFIDENTIAL BUSINESS INFORMATION]. While the AV slowed in response to a nearby vehicle, a work van traveling behind the AV impacted the right rear corner of the AV. Following the contact, the safety driver assumed control of the vehicle and safely pulled over to the side of the road to initiate Motional's incident response protocol. No injuries were reported.
