Annual report, pre-order

State of Commercial Robot Incidents 2026

One document for the year: what went wrong with robotaxis, delivery robots, warehouse fleets, surgical and industrial robots, what happened after the first report, and what the numbers look like once every case is in one place. Built from the 256 cases in the register, each with its sources, and delivered in January 2027 with the fourth quarter data release.

What is inside

  1. The year in numbers: cases by month, category, country and severity, against 2023 to 2025
  2. What happened next: the 44 cases that reached a regulator, a recall, a court or a settlement, with timelines
  3. Company chapters: Waymo, Tesla, Cruise, Zoox and the delivery, warehouse and surgical operators, each with its record for the year
  4. The NHTSA Standing General Order file joined into one row per crash, with the duplicate filings explained, which the public file does not do
  5. Europe and Asia: the first cases found in the local press of ten languages, and what is still missing
  6. Method, limits and everything we got wrong in 2026, with the corrections log
  7. Appendix: every case of the year with its reference, date, company, stage and source count

Delivered as a PDF, laid out for print, around 80 pages. Pre-orders are priced at $149; from release the report costs $249. If it is not delivered by 31 January 2027, every pre-order is refunded in full, no questions. Backers who want to be named are listed in the report.

Everything in the report is also on this site for free. What you are buying is a finished document that can be attached to an underwriting note, a policy brief or a board pack without anyone having to assemble it, and the year of work that keeps the register open.
