May's vehicle was traveling at approximately 20 MPH in a residential neighborhood, and was approximately 1m left of lane center when a black vehicle exited a driveway on the right from behind a fence, facing the road and May's vehicle. This vehicle did not slow down or stop until entering into the roadway and collided with the May vehicle's right side. AVO attempts to swerve to the left but is unable to avoid a collision. The primary factor that led to the collision was the other vehicles action proceeding without the right-of-way into the side of the May vehicle. Upon further inspection, it was determined that we did not predict this collision until it was too late to avoid it even by veering. The velocity of the other vehicle was underestimated and was largely given only yield policies as having the most significant likelihood. There is some room for improvement in our response, which we plan to implement in future software releases, namely, an emergency veer policy must exist that is capable of moving quickly and far enough to possibly avoid the collision, more accurate reporting of velocity for slow-moving agents, estimate acceleration when assessing risk, improvements to predictions of the probability of an agent yielding, and adjusting our risk assessment so that it is heading dependent and more risk would be associated with passing close in front of another vehicle.

On December [XXX], 2024 at 8:28 PM PT a Waymo Autonomous Vehicle ("Waymo AV") operating in Los Angeles, CA was in a collision involving two SUV's, a passenger car, and a pick up truck on [XXX] at [XXX]. The Waymo AV was stopped in autonomous mode facing south at a red light as the lead vehicle in the second from the right lane when an SUV traveling northbound in an oncoming traffic lane ran the red light. As the oncoming SUV entered the intersection, it made contact with a passenger car traveling east on [XXX]. The oncoming SUV continued through the intersection and swerved to narrowly miss a pedestrian before making contact with the front of the Waymo AV and the front driver side of a stationary pickup truck to the right of the Waymo AV. The impact from the oncoming SUV caused the Waymo AV to contact another SUV that was also stationary to the left of the Wayo AV at the red traffic light. The Waymo AV test driver transitioned to manual mode immediately before the oncoming SUV made contact. At the time of the impact, the Waymo AV's Level 4 ADS was not engaged and a test driver was operating the Waymo AV in manual mode. The Waymo AV sustained damage to the front and rear of the vehicle, necessitating a tow from the scene. The oncoming SUV sustained damage to the front and driver side. On November [XXX], 2024 Waymo received notice of an allegation that a Waymo Autonomous Vehicle (Waymo AV) operating in Los Angeles, California may have been in a low-speed collision with a non-motorist (pedestrian) on [XXX] near [XXX]. On November [XXX], 2024 at 4:04 AM PT a Waymo AV operating in Los Angeles, California was traveling northwest on [XXX] when it began to slow to yield to a pedestrian crossing the street ahead from right to left. After the pedestrian had crossed the path of the Waymo AV, the Waymo AV began to accelerate slightly then immediately began to slow again as the pedestrian turned around and approached the Waymo AV. As the Waymo AV was slowing, the pedestrian may have made contact with the driver side of the Waymo AV. At the time of the possible impact, the Waymo AVs Level 4 ADS was engaged in autonomous mode. The pedestrian later claimed to have a minor injury. The Waymo AV did not sustain any damage. Even though this event may involve intentional contact by the pedestrian and the occurrence of actual contact between the Waymo AV and the pedestrian is not clear, Waymo is reporting this event as a crash under Request No. 1 of Standing General Order 2021-01 because a vulnerable road user (pedestrian) influenced the driving task of a vehicle involved and reportedly sustained a minor injury as a result of the event. What this table is and is not. Rows are the operators' own filings, not verified cases, and a filing does not establish fault. NHTSA redacts most Level 2 narratives as confidential business information, so the text column is empty for most Tesla rows; the structured columns are not redacted. When one crash is filed by several entities, an operator and the fleet owner for example, NHTSA gives the filings one same_incident_id; the "one row per incident" view collapses them into a single row at the highest severity any filer alleged, with the count of filings and every report number beside it, so a count of incidents or deaths is not inflated by duplicate filings. The table joins NHTSA's current files (incidents from June 2025, after the third amended order) with its 2021-2025 archive, which used an older layout: severity labels, roadway and weather fields differ between the two sets, and the source_set column in the CSV says which one a row came from. Level 2 reports are shown here because the file contains them, but they are not cases in the register, which counts a vehicle as a robot only when nobody inside is responsible for driving. Source: NHTSA SGO 2021-01. On 7 November 2026 we filed a public comment built on this table in the California Public Utilities Commission's Rulemaking R.12-12-011, asking for machine-readable mileage files and a crosswalk between the Commission's incident categories and the SGO definitions. The themed reading of the driverless narratives is on the reports page.
