The question, then the number

Each page below answers one question in its first paragraph with figures computed from the register's database at the time you read it: NHTSA Standing General Order filings, verified cases, recall notices, and the one rate for which a public mileage series exists. Then the breakdown, how it was counted, and the pages holding the rows behind it. Numbers are CC BY 4.0; cite the page.

Crash reports operators must file under Standing General Order 2021-01. A report is a filing, not a verdict; Level 2 driver-assistance rows are reports, not cases in the register.

How many people have been killed by robotaxis?
The NHTSA Standing General Order file holds 3 reports by operators of driverless systems (ADS) alleging a fatality. The file does not count the dead: a Fatality report means at least one person died in a crash the vehicle was party to, and it does not establish fault or that the automated vehicle struck anyone. The register itself lists 6 verified fatal cases in its autonomous vehicle category, each with a timeline and sources. A further 79 fatal reports in the same file are Level 2 driver-assistance crashes (Tesla Autopilot and Full Self-Driving among them) with a person responsible at the wheel: reports, not robotaxi cases, and not counted above. File refreshed from NHTSA on 2026-11-07.

Verified cases: incidents corroborated across independent sources, recalls traced to their notice.

How many recalls did Zoox have in 2025?
Zoox has 4 recalls on record dated 2025 in The Machine Record, out of 5 recalls across all years. A recall here is a case whose timeline reached a recall notice: for vehicles a defect report filed with NHTSA under 49 CFR Part 573, usually a software update to the automated driving system; for consumer machines a notice from the CPSC or the EU Safety Gate. Separately, Zoox filed 50 crash reports with NHTSA under the Standing General Order for incidents dated 2025; those are crash filings, not recalls, and are counted on their own page.
How many recalls has Waymo had?
Waymo has 6 recalls on record in The Machine Record, the latest dated 2026-06-18. A recall here is a case whose timeline reached a recall notice: for vehicles a defect report filed with NHTSA under 49 CFR Part 573, usually a software update to the automated driving system; for consumer machines a notice from the CPSC or the EU Safety Gate. Separately, Waymo filed 2,399 crash reports with NHTSA under the Standing General Order; those are crash filings, not recalls, and are counted on their own page.
How many delivery robot incidents were there in 2026?
The Machine Record lists 11 verified incidents involving sidewalk and road delivery robots dated 2026: none recording an injury, and 1 that reached a regulator, a recall or a court. A case enters the register only when two independent published sources or one regulator record describe the same event with a date and an outcome, so this is a count of documented incidents, not of everything that happened. The year is still open and the count rises as cases clear verification.
Rates

Rates

Reports divided by miles, only where a public mileage series exists for the same months and state.

What is Waymo's crash rate per million miles?
Waymo's driverless fleet in California filed 1,133 NHTSA crash reports across 133.3 million driverless miles reported to the California Public Utilities Commission between September 2024 and June 2026: 8.50 reports per million miles, of which 0.80 per million allege an injury. This is a rate of filings, not of fault: an operator must report any crash it learns of, including being struck while stopped. For every other operator no public mileage exists, so no comparable rate can be computed; raw filing counts measure who drives the most.
