NHTSA Standing General Order file

How many Cruise robotaxi incidents were reported?

Cruise filed 155 crash reports with NHTSA under Standing General Order 2021-01 over the life of its driverless service, counted at the latest version of each report. Of these, 26 allege an injury and none a fatality. The file as held here was refreshed from NHTSA on 2026-11-07; its latest incident is dated September 2026, and operators have up to a month to file, so the most recent months are still filling.

General Motors filed 149 further ADS reports naming Cruise as the operating entity; where a GM filing describes the same crash as a Cruise filing, the one-row-per-incident view of the table merges them.

A report is the operator's own filing under NHTSA Standing General Order 2021-01. It records that a crash happened and who was involved; it does not establish fault, and most filings describe low-speed contact with nobody hurt.

How this was counted

Counted from NHTSA's Standing General Order files (the current files and the 2021-2025 archive) as loaded into the register, one row per report at its latest version. A report an operator later amended counts once, at its amended severity.

"Injury" means any severity the operator ticked other than unknown, no injury or property damage only; "fatality" is the operator's own Fatality box. Incidents are reports grouped by NHTSA's same_incident_id, so a crash filed by two entities counts once.

The incident date in the current files is given to the month, so counts by month and year follow the month NHTSA published, and a crash on the last day of a month can sit in the next one.

NHTSA's 2021-2025 archive used shorter severity labels (Minor, Moderate, Serious, No Injuries Reported) than the current files; both sets appear in the breakdown as NHTSA wrote them.

Questions

What people ask about this number

How many Cruise robotaxi incidents were reported?
Cruise filed 155 crash reports with NHTSA under Standing General Order 2021-01 over the life of its driverless service, counted at the latest version of each report. Of these, 26 allege an injury and none a fatality. The file as held here was refreshed from NHTSA on 2026-11-07; its latest incident is dated September 2026, and operators have up to a month to file, so the most recent months are still filling.
Does a report mean the vehicle was at fault?
No. Operators must file for any crash their vehicle was involved in, including being struck from behind while stopped. The count is of filings, not of blame; the narrative of each report, where NHTSA has not redacted it, says what happened.
Is this the same as the register's case count?
No. The register's cases are incidents verified from at least two independent sources or a regulator record, mostly the ones with an injury or a consequence. The NHTSA file is every filing the operator made, mostly minor. The two are linked where they describe the same crash.
Can I reuse these numbers?
Yes. The counts are CC BY 4.0: cite The Machine Record with a link to this page. The underlying rows can be downloaded as CSV from the SGO table and the register from the data page.
