Crash data indicates there was a left impact that deployed the front seat airbags and seat belt pretensioners, and that the driver and front passenger were wearing their seat belt at the time of crash. System Data indicates that Super Cruise state changed to Override 1.5 seconds prior to the incident due to steering wheel input and to Disabled/Not Ready 0.1 seconds prior to the crash per system design. On August 31, 2026, GM reviewed the police report that indicated this was a single vehicle incident. The subject vehicle was travelling in the left-most lane and impacted the cement barrier on the left. According to the police report, the vehicle was towed, the passenger was not injured, and the driver had a suspected minor injury but refused treatment.

The customer alleged that he was driving his 2026MY Kia Sorento Hybrid on a highway with the cruise control set at 60 mph when a “vehicle in front suddenly stopped due to a flat tire,” the vehicle allegedly “failed to alert and stop the vehicle automatically,” the driver “realized the vehicle was not going to stop,” and the driver “pull[ed] to the left,” but still hit the stopped vehicle “on the left back corner.” The customer alleged that the airbags deployed and the customer went unconscious. The customer requested to know why the subject vehicle “did not sense the vehicle in front stopped” or “alert and stop the vehicle automatically.”   The same day, KA emailed the customer to request vehicle photos and other information. The customer provided photos of the vehicle and KA advised it would escalate the case for review.   The subject Kia Sorento Hybrid is equipped with Lane Following Assist (“LFA”) and Smart Cruise Control (“SCC”). LFA is a supplemental system that helps to detect lane markings and/or vehicles on the road and assists the driver's steering to help keep the vehicle between lanes. SCC is a supplemental system that detects a vehicle ahead and can help maintain the distance from the vehicle ahead and the set speed. The functionality of LFA and SCC are contingent upon several outside factors, and it is always the responsibility of the driver to safely steer the vehicle, to maintain the vehicle in its lane, and to maintain the vehicle’s speed.   According to vehicle data, the subject Sorento Hybrid had LFA and SCC activated during the 30 seconds pre-crash. At approximately one (1) second pre-crash, the customer applied the brakes, which deactivated the SCC system, then steered approximately 44 degrees to the left before crashing into the front vehicle. In addition, a separate feature (Forward Collision-Avoidance Assist (FCA)), provided a warning to the customer within the one (1) second prior to the crash.   Based on a review of the case report and available information, no issues with the Level 2 ADAS features were identified.

The driver described that cruise control was engaged leading up to the crash event. The driver alleged that, while driving in light rain at 78mph on a freeway in the middle lane, the lane assist engaged unexpectedly and pulled to the right. The driver alleged he attempted to keep the vehicle in the lane by steering to the left, but the lane assist continued to steer to the right, and that this occurred 2-3 times prior to losing control of the vehicle. The vehicle began to veer/coast left and into the center median/ditch, and the vehicle began to spin on the freeway, followed by entering the center median/grass ditch and into a cable barrier and to a stop. The driver alleged that the lane assist pulled the steering wheel while using cruise control and caused the accident. As a result of the crash, the driver alleged that the driver airbag, driver knee airbag, driver seat airbag, driver curtain shield airbag, front passenger seat airbag and passenger curtain shield airbag deployed; the vehicle sustained damage to the front end, the rear end and both sides. Injuries alleged for 3 of 4 occupants: driver (adult male) reports a bruise and cut to left leg; front passenger (adult female) reports cuts on face and both arms, "goose bumps" on head, dizziness and possible concussion, and bruising on her chest from seatbelt and right leg; left rear passenger (12YO female) reports no injuries; and right rear passenger (14YO male) reports laceration on chest from seat belt and broken eyeglasses. An ambulance responded to the scene where first responders wiped the scrapes and confirmed their condition; no other treatment or hospitalization was required. All occupants claimed to be belted; a dog was in a travel crate in the back of the vehicle. The vehicle was towed away from the scene and deemed a total loss. The driver stated the Sharpy County Sherriff Department responded to the crash and provided report number: "SCS2607030021761".

What this table is and is not. Rows are the operators' own filings, not verified cases, and a filing does not establish fault. NHTSA redacts most Level 2 narratives as confidential business information, so the text column is empty for most Tesla rows; the structured columns are not redacted. The table joins NHTSA's current files (incidents from June 2025, after the third amended order) with its 2021-2025 archive, which used an older layout: severity labels, roadway and weather fields differ between the two sets, and the source_set column in the CSV says which one a row came from. Level 2 reports are shown here because the file contains them, but they are not cases in the register, which counts a vehicle as a robot only when nobody inside is responsible for driving. Source: NHTSA SGO 2021-01. The themed reading of the driverless narratives is on the reports page.
