NHTSA publishes the crash reports as three CSV files of 116 columns each, with several versions of the same report mixed in. This is the same data as one table: driverless systems (ADS) and Level 2 driver assistance side by side, 2021 to today including NHTSA's archive, one row per report at its latest version, every row traceable to its NHTSA report number. Free, CC BY 4.0, refreshed from NHTSA on every pipeline run.

Customer reported that while driving, along w/ 3 passengers, on [XXX] in the State of GA, there was a bend in the road & the vehicle didn't make the turn in time & the vehicle ran into wet grass. The customer stated he was unable to recover control of the vehicle once it made contact w/ the wet grass. Customer reported that the vehicle "hit berms, trees, signs & also began to roll until the vehicle settled to a stop." The customer reported he was driving 77mph in a 70mph zone. The customer indicated that Highway Driving Assist (HDA) and Smart Cruise Control (SCC) features were engaged 30 seconds before the incident. The airbags deployed & reported minor injuries of "cuts, scratches, wounds, bumps & bruises". The customer alleged that the lane departure system didn't work properly & wanted a new vehicle or compensation in order to purchase another vehicle. Kia notes that based on available information, the incident occurred in FL. The subject Kia Telluride is equipped w/ HDA, Lane Following Assist (LFA) and SCC. HDA is a supplemental system that helps maintain a set distance & speed from the vehicle ahead when driving on a highway & helps keep the vehicle between lanes while driving. HDA may not be able to recognize all traffic situations & may not detect possible collisions due to limitations of the system. LFA is also a supplemental system that helps to detect lane markings and/or vehicles on the road, & assists the driver's steering to help keep the vehicle between lanes. LFA does not operate at all times & it's the responsibility of the driver to safely steer the vehicle & to maintain the vehicle in its lane. Based on the available information, the customer appears to have a misunderstanding of the capabilities of the vehicle's LFA function & appears to have relied on LFA to steer the vehicle. *1/16/24: Monthly Update Kia is working w/ ODI engineer, Joe Teitelman to schedule a joint inspection of the subject vehicle in Jacksonville, FL. The owner has consented to the EDR being downloaded which will be done when the vehicle is inspected. At the time of this update, the current proposed date for inspection is set for 1/31/24. Kia has no further information at this time. *2/15/24: Monthly Update The joint inspection with ODI engineer Joseph Teitelman occurred on 1/31/24. On 2/12/24, Kia submitted the EDR report w/ a request for confidentiality via NHTSA's SLFTS portal. On 2/14/24, Joseph Teitelman also requested the diagnostic reports from the subject vehicle which was submitted by Kia w/ a CBI request the following day, 2/15/24 via NHTSA SLFTS portal. *3/15/24: Monthly Update Kia engineer reviewed the EDR data previously submitted to NHTSA and identified the following: 1) the vehicle's safety restraint systems operated normally & without fault in each impact 2) the SCC system was engaged before the crash, however, the throttle was being pressed by the driver, overriding the system's operation and 3) the EDR does not record any lane assist function operation, however, the steering angles recorded in the data from -3.0 to 0.0 seconds before crash are above the threshold for what the system can provide both for rate of rotation and total angle. As a result, no vehicle or manufacturing concern was identified and the cause of the crash was most likely due to driver error. |

What this table is and is not. Rows are the operators' own filings, not verified cases, and a filing does not establish fault. NHTSA redacts most Level 2 narratives as confidential business information, so the text column is empty for most Tesla rows; the structured columns are not redacted. The table joins NHTSA's current files (incidents from June 2025, after the third amended order) with its 2021-2025 archive, which used an older layout: severity labels, roadway and weather fields differ between the two sets, and the source_set column in the CSV says which one a row came from. Level 2 reports are shown here because the file contains them, but they are not cases in the register, which counts a vehicle as a robot only when nobody inside is responsible for driving. Source: NHTSA SGO 2021-01. The themed reading of the driverless narratives is on the reports page.
