A large piece of road debris in the right lane caused vehicle #2 to slow and veer onto the right shoulder. A second piece of large road debris was observed by the subject vehicle's operator ("VO") in the center lane approximately 80 meters ahead of the subject vehicle. The VO took control of the truck and began to steer the subject vehicle to the right, but upon seeing vehicle #2 accelerating and re-entering the right lane from the shoulder, the VO instead steered the subject vehicle into the left lane. After passing the debris, the subject vehicle returned to the center lane and continued driving eastbound on the highway. A post-event review of the subject vehicle's cameras showed an ambulance with its emergency lights off (vehicle #3) was traveling in the left lane to the rear of the subject vehicle as the subject vehicle began entering the left lane. When the subject vehicle entered the left lane, vehicle #3 maneuvered onto the left shoulder and came to a stop alongside the median. The subject vehicle made no contact with vehicle #2 or vehicle #3. PlusAI has not been contacted by any road user with regard to this event.

The Waymo AV was traveling northbound on [XXX] in the rightmost lane. A passenger car traveling northbound in the second to the left lane, in front of and to the left of the Waymo AV, began to slow and change lanes to the right when a second passenger car made contact with the rear right side of the first passenger car. The second passenger car continued forward and made contact with an SUV stopped in the queue of traffic in the second from the left lane. As a result of the initial contact with the second passenger car, the first passenger car was pushed east across two lanes of travel, and entered the Waymo AV's lane of travel. The Waymo AV slowed and maneuvered to the right to avoid the oncoming passenger car and the front left side of the passenger car made contact with the left side of the Waymo AV. The Waymo AV rotated to the left and came to rest facing southwest in the second and third from the left lanes. During two subsequent maneuvers in which the Waymo AV pulled forward and to the right in an attempt to navigate off the freeway, the front left corner of the Waymo AV made contact with an "Oversized Load" sign attached to a heavy truck stopped in the second from the left lane. At the time of all impacts, the Waymo AV's Level 4 ADS was engaged in autonomous mode. All involved vehicles sustained damage. The driver of the first passenger car was transported to a hospital by an ambulance with unknown injuries, and the passenger of the Waymo AV alleged minor injuries. The Waymo AV was stopped facing west in the rightmost lane of [XXX] in a queue of traffic for a red light at the intersection with [XXX]. A passenger car, traveling eastbound in the leftmost westbound travel lane at a high rate of speed, entered the intersection against a red light, and made contact with the front left of a second passenger car traveling northbound on [XXX] through a green light. As a result of this contact, the airbags in both passenger cars deployed. The initial eastbound passenger car continued to exit the intersection after contact, proceeded towards the rightmost westbound lane on [XXX], and the front left of the passenger car made contact with the left side of the stationary Waymo AV, resulting in airbag deployment in the Waymo AV. After making contact with the Waymo AV, the passenger car continued eastbound and the left side of the passenger car then made contact with an additional Waymo AV (VIN [XXX]) that was stopped behind the initial Waymo AV in traffic. At the time of the impact, both Waymo AVs' Level 4 ADS were engaged in autonomous mode. The two passenger cars and the initial Waymo AV (VIN [XXX]) sustained damage. An occupant of the northbound passenger car was transported from the scene for medical treatment with unknown injuries. The Zoox vehicle braked hard and attempted to steer left, but contact was unavoidable. The right side front fender, wheels, bodyside, and door panels made contact with the outside edge of the opening door of Vehicle 2. The Zoox vehicle came to an immediate stop and was later towed from the scene to be inspected for damage. First responders arrived at the scene. The passenger of the Zoox vehicle did not report any injuries. Later media stories reported the driver of Vehicle 2 had a "smashed hand" and his "hand was hurting, and that he was also experiencing pain to his neck area and lower back" based on a third party's account, and that the driver "declined their offer to take him to the hospital." Zoox vehicle camera footage shows clearly that no part of the robotaxi came into contact with the driver themselves, and that their hands, leg, and body did not interact with either the robotaxi or the parked vehicle's door at any moment during the contact event.

The Waymo AV remained stopped and four vehicles (two buses, two passenger vehicles) entered [XXX] South from the [XXX] on-ramp and passed the Waymo AV on the left. After the Waymo AV had been stopped for a period of two minutes and 18 seconds, a white SUV entered [XXX] South from the [XXX] on-ramp. After the SUV proceeded past the stop sign onto [XXX] South, it merged into the lane to the left of the Waymo AV. As the SUV entered the lane to the left of the Waymo AV, the front left corner of the SUV made contact with the right side of a pickup truck traveling in the lane to the left of the Waymo AV. The pickup truck then lost control, traveling into the right lane and through the barrier on the rightmost side of the bridge carrying [XXX] over [XXX], then fell to the roadway below. The SUV came to a stop and remained stopped in the lane to the left of the Waymo AV. The passenger of the Waymo AV exited the vehicle after the collision. The Waymo AV proceeded on [XXX] South in autonomous mode. At the time of the impact between the SUV and pickup truck, the Waymo AV's Level 4 ADS was engaged in autonomous mode. The SUV and pickup truck sustained damage. Shortly after the collision, Waymo received notice that minor injuries were reported by passengers of the pickup truck. Waymo is reporting this crash under Request No. 1 of Standing General Order 2021-01 because a vehicle involved was towed away. Waymo may supplement or correct its reporting with additional information as it may become available.

A motorcyclist (Vehicle 2), also southbound on [XXX], was traveling at high speed in the left through lane (lane 3), aggressively approaching a slowing SUV (Vehicle 3) in lane 3. The motorcyclist clipped the right rear corner of Vehicle 3, deflecting the motorcycle to the right, causing the motorcyclist to lose control and fall to the pavement. The motorcyclist was sliding on the pavement when the motorcycle made contact with the Zoox vehicle's rear bumper. The Zoox vehicle operator disengaged from autonomy and brought the vehicle to a stop. The motorcycle came to rest on its right side with the motorcyclist's right leg stuck underneath the motorcycle. An adjacent bystander immediately aided the motorcyclist and asked the Zoox vehicle operator to move the Zoox vehicle "slightly forward". After the Zoox operator moved the vehicle forward, the bystander lifted the motorcycle and the motorcyclist remained on the ground for a short period of time before standing up and walking around the scene. The motorcyclist was observed to have injuries. Police and EMS were called to the scene and the motorcyclist was later transported by an ambulance. The Waymo AV was exiting a parking garage onto [XXX], between [XXX] and [XXX]. As the Waymo AV approached the garage exit, it stopped for a cyclist who was approaching from the left. The Waymo AV remained stationary as the cyclist passed in front of the vehicle, slowed, and appeared to begin to dismount. The cyclist then reached out a hand and made contact with the front passenger side of the stationary Waymo AV, backed the bicycle up slightly, dropped the bicycle, then fell to the ground. At the time of the possible impact, the Waymo AV's Level 4 ADS was engaged in autonomous mode, and a test driver was present. The cyclist received medical attention at the scene and was reportedly transported to a hospital by local emergency services. The Waymo AV did not sustain any damage. Even though this event may involve intentional contact by the bicyclist with a stationary Waymo AV and the occurrence of actual impact between the Waymo AV and the cycle is not clear, Waymo is reporting this event as a crash under Request No. 1 of Standing General Order 2021-01 because a media report included an allegation of a crash and because a vulnerable road user (cyclist) influenced the driving task of a vehicle involved and reportedly was transported to a hospital for medical treatment as a result of the event. The Waymo AV was traveling in the rightmost lane (out of 5 lanes) in the southbound direction of [XXX], and the left 3 lanes were closed due to a work zone. The vehicle traveling ahead in the same lane as the ADV made an abrupt lane change to the left, revealing a stack of slow-moving vehicles in the Waymo's lane. The test driver of the Waymo AV took manual control of the vehicle and started to decelerate and steer toward the right shoulder in response to slow traffic ahead. A passenger vehicle approached the Waymo AV from behind and made contact with the rear of the Waymo AV. At the time of the impact, the Waymo's Level 4 ADS was not engaged and a test driver (in the driver's seating position) was operating the Waymo AV in manual mode. The Waymo AV and the other vehicle sustained damage. Transdev Alternative Services is reporting this collision under the 5-day reporting guidelines because a vehicle was towed but nobody was transported to the hospital from the scene of the collision and there was no Vulnerable Road User involved. Transdev may supplement or correct its reporting with additional information as it may become available. This is a duplicate of collision report ID: 30270-5650-1.

Transdev may supplement or correct its reporting with additional information as it may become available. This report is a duplicate to WaymoLLC-202111[XXX]-0 (rev.202111[XXX]201639).

What this table is and is not. Rows are the operators' own filings, not verified cases, and a filing does not establish fault. NHTSA redacts most Level 2 narratives as confidential business information, so the text column is empty for most Tesla rows; the structured columns are not redacted. When one crash is filed by several entities, an operator and the fleet owner for example, NHTSA gives the filings one same_incident_id; the "one row per incident" view collapses them into a single row at the highest severity any filer alleged, with the count of filings and every report number beside it, so a count of incidents or deaths is not inflated by duplicate filings. The table joins NHTSA's current files (incidents from June 2025, after the third amended order) with its 2021-2025 archive, which used an older layout: severity labels, roadway and weather fields differ between the two sets, and the source_set column in the CSV says which one a row came from. Level 2 reports are shown here because the file contains them, but they are not cases in the register, which counts a vehicle as a robot only when nobody inside is responsible for driving. On 7 November 2026 we filed a public comment built on this table in the California Public Utilities Commission's Rulemaking R.12-12-011, asking for machine-readable mileage files and a crosswalk between the Commission's incident categories and the SGO definitions. The themed reading of the driverless narratives is on the reports page.
