The vehicle operator (AVO) presses on the accelerator, taking control of the Ego, then accelerates into the curb at the stop. The Ego made contact with the curb 3 seconds after switching into manual, causing severe damage to the front-right tire. Resimulation of this event shows that there were no autonomy errors that led to this event, and had the Ego stayed in autonomy, this event would not have happened. This AVO had just completed our training course with only a few hours of service time before this incident. Upon further review of the AVO performance, on their own, it was decided to remove her from the AVO role. |

The Autonomous vehicle operator took over control of the vehicle and attempted to reverse, but was unable to get out of the way of the reversing vehicle. The planner did roll out the agent with a reversing policy and predicted the collision, but the system currently is not able to honk or reverse or do anything else that could have avoided this. The primary factor was fault by the lead vehicle. There was no fault in our software that led to this event. |

As the Ego proceeded, the system initially recognized the static retaining wall as a large, non-rectangular object. As the Ego approaches the wall, the resulting density of lidar returns on the object (wall) exceeds the detectors maximum limitation. The newer lidar points, toward the forward edge of the elongated object, exceeding the limit were filtered out, causing the track to disappear when the ego approached 5m from the track (i.e. the wall). The situation was exacerbated by the fact that the road edge was incorrectly marked in that section. The road edge was drawn through the retaining wall and thus the vehicle did not have any prior knowledge about the edge of the road. As a result, the Ego drove too close to the wall, scraping it at a speed of 2 meters per second while driving straight (i.e. in lane-nominal policy) with no nudge or veer corrections. There was no damage to the wall. The limitation leading to the lost track affects Software versions v26.0 (when the Open World Tracklet v2 feature was introduced) through v31.0. This is the only known instance of this failure mode, and in v32.0 an optimization was made to remove the LIDAR threshold entirely and mitigate the vulnerability. Furthermore, the incorrect road edge annotation was corrected shortly after this incident and the map updated for the deployed vehicles. |

The driver described that, while driving on a rural highway with adaptive cruise control engaged and passing a turnoff on the left side, a malfunction warning appeared that said "adaptive cruise control malfunction," and the vehicle pulled hard to the left and the steering locked. As the driver attempted to straighten the vehicle, it impacted the center cable divider. The driver alleged the steering wheel then released and caused the car to steer across two lanes of traffic to the right and the vehicle rolled. As a result of the crash, the driver alleged that: the curtain airbags deployed; there was damage to the left side and top of vehicle including a broken driver's window and rear windscreen; the driver suffered from a concussion and an occupants left arm was cut, both treated at an emergency room; the vehicle was towed away from the scene. The rental company stated the [XXX] responded to the crash and provided report numbers "[XXX]". On October 21, 2025, Toyota communicated directly with the driver of the vehicle. The driver provided additional information indicating that Lane Trace Assist feature was engaged at the time of the incident. ~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~ Update report (January 13, 2026): On January 9, 2025, Toyota completed vehicle inspection field report related to the crash event and obtained the Event Data Recorder (EDR) record and PCS Freeze Frame Data (PCS FFD) from the crash event. The PCS FFD showed that dynamic radar cruise control (DRCC) was engaged in the recorded moments leading to the crash event. Toyota is investigating the Lane Tracing Assist (LTA) engagement status, and the timing of driver inputs related to steering, acceleration, and braking. This information will be provided in a future update. Minor revisions to information in the report fields above were made based on factual information obtained from the field report and EDR record. ~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~ Update report (January 14, 2026): No materially new information is available at this time. Submitting update report to correct an error in [XXX]. ~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~ Update report (February 13, 2026): On February 12, 2025, Toyota completed their assessment of the Lane Tracing Assist (LTA) engagement status, and the timing of the driver inputs related to steering, acceleration, and braking. The PCS Freeze Frame Data (PCS FFD) consistent with the crash event was reviewed. The Lane Tracing Assist (LTA) turned off approximately two seconds prior to left steering inputs followed by the vehicle drifting off the roadway to the left. Right steering input began and brakes were applied briefly disabling the Dynamic Radar Cruise Control (DRCC). The right steering inputs continued as the vehicle began to roll over. Based on this information, Toyota determined that the Level 2 ADAS was not engaged in time leading up to the crash. Therefore, this will be the final update for this incident. |

The vehicle was traveling east on [XXX] near the Meadow Vista, CA exit, 1hr 44min into the trip, when the crash event occurred. In the moments leading up to the crash the vehicle was in an ADAS L2 state, with Lane Tracing Assist (LTA) and Dynamic Radar Cruise Control (DRCC) functions engaged and travelling at 82mph. During a left hand curve, the vehicle was centered in the #1 lane (4.65 sec prior to the crash), when the driver induced a steering input (1.7Nm) to the right (3.15 sec prior to the crash). The vehicle crossed the lane line into the #2 lane which caused the LTA feature to cancel and disengage (2.40 sec prior to the crash). The vehicle departed the #2 lane to the right shoulder (1.05 sec prior to the crash). The vehicle continued drifting to the right without driver steering input before the right side tires exited the pavement and the car pulled to the right, at which the driver applied left steering input (0.30sec). The vehicle went up the slope to the right side and moved left back into the roadway ending the crash event. Based on this data, the initial driver input caused the vehicle to exit the lane, which caused the ADAS L2 to disengage, and the driver did not make steering inputs to attempt to regain control of the vehicle until the tires went off the shoulder, 0.30sec prior to the crash. Also, it was observed that the customer installed a windshield decal (banner style at top) that was partially obstructing the forward camera; this can affect the ability of the camera to recognize lanes. This is warned against in the owner's manual which states, "Warning: To prevent malfunction of the front camera: do not attach stickers (including transparent stickers) or other items to the area of the windshield in front of the front camera" (The OM has an illustration of the windshield with shaded area representing area to avoid). This will be the final update for this incident. |

What this table is and is not. Rows are the operators' own filings, not verified cases, and a filing does not establish fault. NHTSA redacts most Level 2 narratives as confidential business information, so the text column is empty for most Tesla rows; the structured columns are not redacted. The table joins NHTSA's current files (incidents from June 2025, after the third amended order) with its 2021-2025 archive, which used an older layout: severity labels, roadway and weather fields differ between the two sets, and the source_set column in the CSV says which one a row came from. Level 2 reports are shown here because the file contains them, but they are not cases in the register, which counts a vehicle as a robot only when nobody inside is responsible for driving.
