While navigating northbound on [XXX], vehicle 004D approached the intersection of [XXX] under automated control. As vehicle 004D proceeded through the intersection, a 2020 Nissan Pathfinder navigating eastbound on [XXX] entered the intersection and struck vehicle 004D on the drivers door. Vehicle 004D had right of way through the intersection with no stop or yield the other vehicle had been stopped at a stop sign prior to the collision. There were no injuries reported at the scene, and both vehicles were driven away from the site of the collision. Prior to the collision, the safety operator in vehicle 004D stated that they noticed the other vehicle entering the intersection, applied the vehicles brakes, and steered away from the path of the other vehicle. Though they were able to bring vehicle 004D to a complete stop, they were unable to completely avoid the collision. Review of dashcam footage and of CCTV footage covering the intersection corroborate this narrative. The safety operator was referred for a post-accident drug and alcohol testing after the incident. They tested negative for any impairment. Analysis of the ADS logs determined that the ADS had detected the potential collision and had planned an emergency braking action. The safety operators intervention revoked control of the vehicle from the ADS preventing any automated braking actions. The [XXX] responded to the accident scene. The driver of the other vehicle was cited for failing to stop at the stop sign, and the JSO CSO noted that vehicle 004D was not at fault for the accident.

Customer …Customer contacted Kia via Web Note on June [XXX], 2025, and reported he was involved in an accident while driving his 2024MY Kia Sportage with Adaptive Cruise Control (ACC) enabled at approximately 40-45 MPH. Customer claimed that another vehicle was stopped at a traffic light and the subject vehicle did not slow down as expected. Customer then attempted to disengage the ACC and manually apply the brakes, but the system failed to respond, preventing me from stopping the vehicle. Customer added that the ACC system did not reduce speed appropriately or allow me to take manual control in [REDACTED, MAY CONTAIN CONFIDENTIAL BUSINESS INFORMATION] to avoid the crash. Customer reported there was a significant collision, during which the airbags deployed. He confirmed that he was able to drive the vehicle away from the scene. Customer reported sustaining injuries to his thumb and stomach because of [REDACTED, MAY CONTAIN CONFIDENTIAL BUSINESS INFORMATION] seat belt and requested warranty coverage, a vehicle replacement, and compensation. The customer confirmed they were not transported to the hospital and is not currently under any medical attention. Kia Customer Care (CC) contacted customer to obtain additional information regarding the incident. Customer reported that Highway Driving Assist (HDA), Smart Cruise Control (SCC) and Lane Following Assist (LFA) were also on at the time of the incident and provided post-incident photos of the vehicle. The photos provided by the customer show damage to the center portion of the front bumper, consistent with contact against the rear tow hitch on the other vehicle which is also visible in the photos. Kia escalated the case for further review. Kia confirmed that both SCC and LFA were engaged at the time of the crash. While the customer reported that HDA was also active, Kia confirmed that HDA was not engaged at the time of the crash. Kia also reviewed the ECU data which confirmed that the brake pedal was pressed two seconds before impact, which disengaged the SCC system. The data also indicates that the Forward Collision-Avoidance Assist (FCA) system detected a vehicle ahead and issued a Level 1 audible and visual warning prior to the collision. These data points suggest that the driver did not brake early or forcefully enough to avoid the collision. The subject Kia Sportage is equipped with HDA, LFA, and Smart Cruise Control (SCC). Another name for SCC is Adaptive Cruise Control, which is how the customer refers to the system. LFA is a supplemental system that helps to detect lane markings and/or vehicles on the road and assists the drivers steering to help keep the vehicle between lanes. The functionality of LFA is contingent upon several outside factors and it is always the responsibility of the driver to safely steer the vehicle and to maintain the vehicle in its lane. Further, post-impact drive cycle data shows no braking concerns. The vehicle was driven away from the scene without reported braking issues, and the brake system continued to function as expected. Kia is currently working to obtain additional information to evaluate this incident. *** 8/15/2025 Monthly Update *** On July 15, 2025, Kia received a request from Mr. Teitelman via email for materials relating to the incident report. On July 23, 2025, Kia provided Mr. Teitelman the requested information via NHTSA's SLFTS portal. In addition, the customer has since stated he is unable to provide any additional information because he sold the vehicle to his insurance provider. Kia has no further information.

The owner described that cruise control was engaged leading up to the crash event, and alleged that while driving on a highway, the car's "lane assist" could not read the lane lines properly and pulled the vehicle. The owner tried to correct the vehicle by steering and braking, but the vehicle went off the road and collided with a bush that stopped the vehicle. The owner alleged the brakes did not work properly. As a result, the vehicle had 3 damaged tired and wheels, brake system damage, bumper damage, bottom cover damage, and scratches to the paint. The owner stated the crash occurred between 6:00-7:00AM. Additionally, the owner alleged that the previous day, the "cruise control did not function properly by having to jerk it as it was not recognizing hands were on the steering wheel."

What this table is and is not. Rows are the operators' own filings, not verified cases, and a filing does not establish fault. NHTSA redacts most Level 2 narratives as confidential business information, so the text column is empty for most Tesla rows; the structured columns are not redacted. The table joins NHTSA's current files (incidents from June 2025, after the third amended order) with its 2021-2025 archive, which used an older layout: severity labels, roadway and weather fields differ between the two sets, and the source_set column in the CSV says which one a row came from. Level 2 reports are shown here because the file contains them, but they are not cases in the register, which counts a vehicle as a robot only when nobody inside is responsible for driving. Source: NHTSA SGO 2021-01. The themed reading of the driverless narratives is on the reports page.
