NHTSA publishes the crash reports as three CSV files of 116 columns each, with several versions of the same report mixed in. This is the same data as one table: driverless systems (ADS) and Level 2 driver assistance side by side, 2021 to today including NHTSA's archive, one row per report at its latest version, every row traceable to its NHTSA report number. Free, CC BY 4.0, refreshed from NHTSA on every pipeline run.

Ford was not previously aware of the incident and was subsequently told that there were two fatalities and that the driver of the Mach-E alleged that BlueCruise was engaged at the time of the incident. A police report obtained by Ford on April 11, 2024 states that the accident occurred in Philadelphia, Pennsylvania on March [XXX], 2024 at 3:20 a.m. in the northbound lanes of [XXX] near mile marker 27.4. The report states that a disabled 2006 Toyota Prius was stopped in the left lane of traffic and that a 2012 Hyundai Elantra had stopped behind the Prius. The report further states that a 2022 Mach-E traveling in the left lane struck the Elantra from behind and that this initial collision pushed the Elantra forward, causing it to strike the Prius. The report states that the driver of the Hyundai and the driver of the Prius (who had exited the vehicle and was classified as a pedestrian) both sustained fatal injuries from the crash. Based on further investigation, the following reflects Ford's understanding of the responsive facts and circumstances regarding this incident. On Sunday, March [XXX], 2024, about 3:20 a.m. eastern standard time, a 2022 Ford Mustang Mach-E electric-powered sport utility vehicle was traveling northbound in the left lane on [XXX] ([XXX]) in Philadelphia, Pennsylvania. The Mach-E was equipped with advanced driver assistance systems, including those providing partial automation capabilities that Ford refers to as "BlueCruise". Vehicle-based data obtained post-crash indicates that the vehicle's 23-year-old driver was using BlueCruise as it approached mile marker 26.8. At this location, [XXX] consisted of six traffic lanes, three each in the north- and southbound directions. The opposing lanes were divided by a concrete median barrier and the posted speed limit was 55 mph. some roadway lighting was present in this section of [XXX]. The Mach-E encountered two passenger vehicles that were stopped in the travel lanes of [XXX]. The Mach-E collided with the rear of the stationary 2012 Hyundai Elantra, which was attended by a 20-year-old driver. The Hyundai then collided with the rear of the stopped 2006 Toyota Prius, attended by a 21-year-old driver. At the time of the crash, the Hyundai driver was exiting the vehicle while the Toyota driver was standing near the front of the Hyundai. As a result of the crash, the drivers of both the Hyundai and Toyota were fatally injured. The Mach-E driver sustained minor injuries. At the time of the crash, the weather was clear and road surface was dry. Ford is informed of a pending police investigation regarding the Mach-E driver's potential impairment. |

The passenger car (Toyota Tacoma, equipped with front grille/brush guard) approached from behind, honked the horn, and struck the stationary subject vehicle. The driver of the subject vehicle proceed through the intersection and pulled over into an empty parking lot to safely assess the incident. The passenger car is noted as remaining at the light for an additional cycle of the traffic light before driving off and not pulling over with the vehicle it just struck. The subject vehicle's 3 occupants reported feeling the impact but report no injuries, airbag deployments, or tow-aways. The subject vehicle's damage was to the right rear bumper, including the right rear sensor and mounting bracket. |

I was in the left-hand lane going east. The Volvo was in pilot assist mode or at least I was under the impression that it was in pilot mode and I was briefly, looking at the central infotainment screen with one eye, while the other eye was on the road when the car passed that median on the left side of the vehicle instead of following the road to the right, the car veered to the left or kept going straight depending on your perspective by the time that I realized which was pretty much instantly that the car was not acting properly, I wrestled the steering wheel to the right, but it was not in time to prevent the vehicle from striking a oncoming car. This oncoming car was the Mazda, which I struck with the front left corner of the Volvo hit the Mazda front left corner which bounced the Volvo into the other opposing traffic vehicle, a Honda, which was in the right lane, causing minor damage to the Honda. At this point the vehicle came to arrest in front of the sidewalk . Note that the road was clear it was sunny, and the road markings were highly visible. The driver of the Mazda was injured mildly from his airbag. The driver of the Honda was injured, and myself was also uninjured." Volvo Car Review: We are unable to determine if Pilot Assist was engaged. ACC was on and engaged. Volvo Car review of the incident: The lane markings disappeared due to the intersection at the same time the road bends. The driver failed to monitor the Pilot Assist through the intersection and provide the correct directional input to the vehicle. The driver failed to maintain the correct lane and crossed into oncoming traffic and stuck the vehicle driving westbound. Pilot Assist might deactivate automatically and resumes without prior notice. This is not a collision avoidance system. The driver is always responsible and must intervene if the system fails to interpret how the lanes go due to lack of road line markings. Pilot Assist is not to be used in intersections. Statement from the Customer: I was briefly, looking at the central infotainment screen The driver was distracted just before the accident and wasnt actively monitoring the road conditions or the Pilot Assist system that always requires driver attention and hands on steering wheel. |

On February [XXX], 2024 at 2:34 PM PT a Waymo Autonomous Vehicle (Waymo AV) operating in San Francisco, California was in a collision involving a bicyclist on [XXX] at [XXX]. The Waymo AV was traveling westbound on [XXX] and came to a stop at a 4-way stop sign intersection at [XXX]. When it was the Waymo AVs turn at the intersection, the Waymo AV entered the intersection to proceed straight. At this point, a delivery truck traveling eastbound on [XXX] was already in the middle of the intersection (the delivery truck arrived at its stop sign first and proceeded into the intersection before the Waymo AV). As the Waymo AV passed the rear of the delivery truck, an occluded bicyclist who was traveling behind the delivery truck became visible and was making a sharp left turn onto northbound [XXX], in front of the Waymo AVs path. The cyclist had entered the 4-way stop intersection without stopping or yielding. The Waymo AV detected the bicyclist, applied heavy braking, but was unable to avoid contact with the bicyclist before coming to a full stop. At the time of the impact, the Waymo AVs Level 4 ADS was engaged in autonomous mode. The Waymo AV sustained minor body damage, and the bicyclist reported minor injuries and damage to their bicycle. The Waymo AV remained stopped in the intersection for approximately one minute, and after the path was clear, resumed driving for a short period of time. The Waymo AV was then returned to the scene by one of Waymos roadside assistance specialists. The Waymo AV did not require towing and was returned to the depot in manual mode. Waymo is reporting this crash under Request No. 1 of Standing General Order 2021-01 because a vulnerable road user was involved. Waymo may supplement or correct its reporting with additional information as it may become available. |

What this table is and is not. Rows are the operators' own filings, not verified cases, and a filing does not establish fault. NHTSA redacts most Level 2 narratives as confidential business information, so the text column is empty for most Tesla rows; the structured columns are not redacted. The table joins NHTSA's current files (incidents from June 2025, after the third amended order) with its 2021-2025 archive, which used an older layout: severity labels, roadway and weather fields differ between the two sets, and the source_set column in the CSV says which one a row came from. Level 2 reports are shown here because the file contains them, but they are not cases in the register, which counts a vehicle as a robot only when nobody inside is responsible for driving. Source: NHTSA SGO 2021-01. The themed reading of the driverless narratives is on the reports page.
