The SGO file, in a form you can search

NHTSA publishes the crash reports as three CSV files of 116 columns each, with several versions of the same report mixed in. This is the same data as one table: driverless systems (ADS) and Level 2 driver assistance side by side, 2021 to today including NHTSA's archive, one row per report at its latest version, every row traceable to its NHTSA report number. Free, CC BY 4.0, refreshed from NHTSA on every pipeline run.

The vehicle impacted a police vehicle that was on the side of the road (see update). The driver was transported to the hospital and the vehicle was towed. Rivian believes the driver falling asleep was the cause of the accident and the HWA system was not a factor. Update: Rivian is still working with the Tennessee Highway Patrol (THP) and the local DA to gain access to the Rivian vehicle for further investigation and assessment. The condition of either vehicle involved remains unknown and many details are still unavailable. We have learned that the THP vehicle that was impacted was in pursuit, or about to be in pursuit, of a vehicle traveling the wrong way down the highway. It may not have been parked on the side of the road as initially believed. So, the pre-crash movement still remains unknown but there is potential for a very unusual maneuver such as a sudden U-turn on the highway, rapidly reentering the lanes of travel from the shoulder, a vehicle traveling the wrong way at high speed, etc. to have been a factor. The subject Rivian vehicle is an early VIN Gen 1 vehicle equipped with Highway Assist, not the Enhanced Highway Assist recently released to Gen 2 vehicles. The investigation is ongoing.

At 6:45:30 PM there was a significant steering input from the driver that overrides HWA, which transitions the vehicle into Adaptive Cruise Control (ACC). The vehicle continued in ACC for approximately 1 second, while drifting out of the lane to the left, until accelerometer data shows the vehicle comes into contact with an object at 6:45:31 PM ET. The vehicle then comes to a full rest about 6 seconds later. Just over one hour later, at 8:04 pm ET, the customer called Rivian asking for information about long distance towing. Information was provided but a tow was not requested by the customer or arranged by Rivian and it is unknown if the vehicle was actually towed. The customer did not provide any additional information about the nature of the crash or make any allegations that the performance of the vehicle was a factor. Rivian believes that the driver failing to take control of the vehicle after they cancelled HWA was the cause of the crash and vehicle performance was not a factor.

What this table is and is not. Rows are the operators' own filings, not verified cases, and a filing does not establish fault. NHTSA redacts most Level 2 narratives as confidential business information, so the text column is empty for most Tesla rows; the structured columns are not redacted. The table joins NHTSA's current files (incidents from June 2025, after the third amended order) with its 2021-2025 archive, which used an older layout: severity labels, roadway and weather fields differ between the two sets, and the source_set column in the CSV says which one a row came from. Level 2 reports are shown here because the file contains them, but they are not cases in the register, which counts a vehicle as a robot only when nobody inside is responsible for driving. Source: NHTSA SGO 2021-01. The themed reading of the driverless narratives is on the reports page.
