According to the customer,…The customer/driver of the subject vehicle notified Volkswagen Group of America (VWGoA) of an incident that occurred while traveling on the highway with travel assist (SAE L2 system) activated. According to the customer, they unexpectedly lost control of the vehicle in which it veered to the right, impacting the concrete wall / sound barrier. Prior to impact, the customer stated they were driving in a marked construction zone, in the left lane, and it had recently began raining with the surface of the road noted as wet, but not pooling. To the left of the customer was a concrete median barrier, dividing the highway, and to the right was the right driving lane, then a narrow shoulder with construction barrels and the concrete wall / sound barrier. The frontal impact between the vehicle and the sound barrier resulted in the airbags deploying and then the vehicle coming to a rest in the right lane of the highway. There were no reported injuries, no additional road users involved, and police were notified and provided a report as well as a citation to the driver. The vehicle was towed from the scene and is currently in possession of the insurance company. VWGoA plans to further investigate this matter and will submit subsequent reports accordingly. Update Report 8/15/2024: VWGoA conducted an inspection of the subject vehicle and information from the airbag control module (ACM) identified a speed of 70 to 68mph from -5.0 to -3.5 seconds precrash. After -3.0 seconds, the report reflects ABS and ESC activity all the way until 0.0 seconds suggesting a loss of control, possibly from the weather event reported by the customer or driver inputs. The front radar module was no longer with the vehicle at the time of the inspection and no EDR or ASM (Assist System Monitor) data was able to be obtained. The precise cause of the crash could not be determined, but no manufacturing shortcoming was identified. |

On July [xxx], 2024, at 12:59 PM PT a Waymo Autonomous Vehicle ("Waymo AV") operating in [XXX], California was involved in an alleged collision with a non-motorist (cyclist) at a parking garage exit on [XXX] between [XXX] and [XXX]. The Waymo AV was exiting a parking garage onto [XXX], between [XXX] and [XXX]. As the Waymo AV approached the garage exit, it stopped for a cyclist who was approaching from the left. The Waymo AV remained stationary as the cyclist passed in front of the vehicle, slowed, and appeared to begin to dismount. The cyclist then reached out a hand and made contact with the front passenger side of the stationary Waymo AV, backed the bicycle up slightly, dropped the bicycle, then fell to the ground. At the time of the possible impact, the Waymo AV's Level 4 ADS was engaged in autonomous mode, and a test driver was present. The cyclist received medical attention at the scene and was reportedly transported to a hospital by local emergency services. The Waymo AV did not sustain any damage. Even though this event may involve intentional contact by the bicyclist with a stationary Waymo AV and the occurrence of actual impact between the Waymo AV and the cycle is not clear, Waymo is reporting this event as a crash under Request No. 1 of Standing General Order 2021-01 because a media report included an allegation of a crash and because a vulnerable road user (cyclist) influenced the driving task of a vehicle involved and reportedly was transported to a hospital for medical treatment as a result of the event. Waymo may supplement or correct its reporting with additional information as it may become available. |

It signals the lane change appropriately early and begins to gradually decelerate for the change. The trailing SUV continues to accelerate while the May vehicle is simultaneously decelerating, and eventually changes lanes directly into the May vehicle. Trailing SUV drives away without slowing meaningfully, disappearing around an upcoming turn. May vehicle deceleration never exceeds -1.3m/s2, so there is no plausible reason a responsible driver would not have had adequate time to adjust their speed or path to avoid a collision. |

Please see reports 855-8237-(1 and 2) to see prior updates. 9/13/2024 Update to 855-8237-2: MBUSA conducted a scene and vehicle inspection on August 14, 2024, which included a physical examination of the vehicle. The subject vehicle was located in a salvage yard. The subject vehicle was left uncovered, exposing the vehicle to rain and other weather conditions (including heavy rains associated with Tropical Storm Debby). A significant amount of standing water was observed in both the driver and passenger footwells. As a result of this water damage to the subject vehicle, MBUSA was not able to establish a connection to the vehicle system diagnostics, control units, or EDR. A physical inspection of the braking and steering systems was subsequently performed. This inspection did not identify evidence of any steering or braking malfunction and the available information indicates both systems operated as intended at the time of the incident. The dashcam videos provided by the owner show the driver increasing the vehicles speed from 35mph at the start of the video to 46.5mph at the time of the crash. No audible warnings or steering wheel movement were observed in the driver-facing video. When DISTRONIC with Active Steering Assist is active, the vehicle is designed to allow a driver to overrule and suppress the system by depressing the accelerator pedal. Based on the speed increase observed in the provided videos, it suggests that the system was not providing speed control to the subject vehicle prior to the incident. Based on its investigation and review of the videos provided by the vehicle owner, MBUSA has concluded that that the vehicle operated as intended and that DISTRONIC with Active Steering Assist was likely not engaged during the 24-second period captured by the dashcam video recording and when the driver lost consciousness. MBUSA has concluded its investigation into this matter. |

The incident reportedly took place at the intersection of [XXX] in Shelbyville, TN, both of which are not limited access highways. On [XXX], 2024 Nissan determined that ProPilot Assist 1.0 was active ("hands-on" mode) within 30 seconds of the incident. Telematics data was last transmitted approximately 1,500 ft before the intersection. No crash notification was transmitted at the time of the incident. Based on information in the news report, on Monday [XXX], 2024, the driver of the Ariya was traveling north on [XXX]. A tractor-trailer that was reportedly traveling west from [XXX] pulled out in front of the Nissan Ariya. According to the report the Ariya traveled underneath the trailer and continued approximately 100 yards before coming to a stop. The driver and passenger of the Ariya reportedly sustained fatal injuries. The tractor-trailer driver was reported not injured. According to the police report, at approximately 6:15 p.m. on July [XXX], 2024, the Nissan Ariya was traveling north on[XXX] in the number two lane when a tractor trailer traveling west bound on[XXX] attempted to make a left-hand turn onto [XXX] South, traveling across the northbound lane. The police report states that the Nissan Ariya collided with the left rear side of the trailer and continued traveling north into the median. According to the police report: the Ariya driver and passenger sustained fatal injuries and had to be extricated from the vehicle multiple airbags were deployed the passenger's seat belt use was "improper" the Ariya driver was belted and the tractor trailer driver was operating with a disqualified license. Nissan obtained vehicle EDR from law enforcement. That data indicated that the speed at impact was 53.4 mph, with no pre-impact brake application. Nissan conducted a preliminary, non-destructive inspection on August [XXX], 2024. The vehicle had been stored in the open air and uncovered until August [XXX], 2024. Various components were stored by the junkyard in an unidentified location and made available for inspection. The vehicles condition is consistent with underride of the tractor trailer and extrication. The driver seatbelt was found buckled with the lap portion routed behind the seat, the pretensioner having activated and shoulder-belt webbing cut near the d-ring location. The passenger seatbelt was buckled, with both lap and shoulder belt pulled tight against the seat back, with the pretensioner having activated. Nissan was able to collect the vehicles ADAS control unit and Lane Camera assembly. Nissan was unable to download additional data from either system due to the nature of the incident which resulted in an immediate loss of supplied power to the assemblies, and consequently prevented event data from being stored in memory. Nissan conducted an inspection of the trailer on October 9th. No findings during the inspection changed our understanding of the incident. Nissan has identified no evidence that suggests that the incident was attributable to any defect or improper functioning of any of the vehicle's safety system. Nissan has concluded its investigation into this incident. | |

What this table is and is not. Rows are the operators' own filings, not verified cases, and a filing does not establish fault. NHTSA redacts most Level 2 narratives as confidential business information, so the text column is empty for most Tesla rows; the structured columns are not redacted. The table joins NHTSA's current files (incidents from June 2025, after the third amended order) with its 2021-2025 archive, which used an older layout: severity labels, roadway and weather fields differ between the two sets, and the source_set column in the CSV says which one a row came from. Level 2 reports are shown here because the file contains them, but they are not cases in the register, which counts a vehicle as a robot only when nobody inside is responsible for driving. Source: NHTSA SGO 2021-01. The themed reading of the driverless narratives is on the reports page.
