NHTSA publishes the crash reports as three CSV files of 116 columns each, with several versions of the same report mixed in. This is the same data as one table: driverless systems (ADS) and Level 2 driver assistance side by side, 2021 to today including NHTSA's archive, one row per report at its latest version, every row traceable to its NHTSA report number. Free, CC BY 4.0, refreshed from NHTSA on every pipeline run.

("VW") filed an Incident Report (NHTSA-provided Report ID-1420-7914- the "Report") under the Second Amended Standing General Order 2021-01, dated April 5, 2023 (the "SGO"). The subject vehicle referred to in the Report contains the Reporter's software and hardware. Since the Reporter is also a Reporting Entity under the SGO, the Reporter hereby submits this report. In order to avoid duplications and additional CBI requests, this submission includes the publicly available information of the Report and incorporates the Report by reference. This submission is based on the information provided to the Reporter by VW. The Reporter currently believes that it has no independent knowledge of the information in this report. From Volkswagen: The subject vehicle (autonomous vehicle - AV) was proceeding through a parking garage while engaged in autonomous driving (AD) mode. The AV detected an oncoming passenger vehicle (PV), made adjustments to its route by moving to the right, and slowed to a stop to provide additional space for the PV. As the PV passed, it attempted to make a left-hand turn behind the AV, but the PV's driver side mirror scraped the rear driver's side LiDAR housing of the AV. The passenger vehicle continued driving without stopping. After noticing the contact, the safety driver of the subject vehicle manually disengaged AD mode to assess the incident. There were no reported injuries, airbag deployments, or tow-aways. The damage to the subject vehicle was a scuff to the driver's side Short Range LiDAR trim cover. VWGoA does not plan to investigate the incident any further as the facts and circumstances do not indicate any issue with the vehicle's performance. |

On 6/28/24 claimant notified Nissan Consumer Affairs of an accident which reportedly occurred on 5/[XXX]/24. The claimant states that ProPilot Assist was in use and did not stop the vehicle. Claimant states he was "prevented from braking," explaining that the brakes had failed. Claimant reports neck pain and a small scratch on right thumb and finger, but did not seek medical attention. Claimant stated the complaint is related to brake failure and claimant is not reporting a failure of the ProPilot Assist performance. Nissan inspected the subject vehicle in connection with the brake failure complaint and found the vehicle's braking system to be fully functional. No related DTCs were noted. AEB warning lamp was off, indicating a fully functional system. EDR data shows the vehicle was traveling at 64.6mph prior to the brake application applied 3 seconds before time zero. Vehicle data shows that prior to impact, vehicle speed had been reduced to below 20 mph, with AEB activating max braking 0.5 second before the crash. Of particular note, the driver was issued a citation by police for following a vehicle too closely as he was approaching an intersection with a 25 deg right hand turn and the EDR system does not record the status of PPA system. According to the vehicle investigation report, the subject vehicle was towed from the incident scene. Nissan denied the claim because there is no evidence of any manufacturing or product defect that would have caused or contributed to this incident. On July 22, 2025, during review of the vehicle inspection report for this incident in preparation of Nissan's response to EA25-001, Nissan determined that this incident was reportable under the criteria of the Second Amended Standing General Order 2021-01 and is therefore submitting this 5-day and Final Report. | |

The owner alleged that she, "put her car in self drive mode while it was raining, and the vehicle did not detect the big truck and when she went to brake, it pushed into the truck, which slid into another vehicle." The customer further clarified her allegation to be, "the Automatic Collision System did not work properly" and, "the vehicle misjudged the stop range and the PCS did not alert her, so she rear ended the vehicle in front of her." The owner stated that after the crash, the Toms Rivers Police Department (New Jersey) responded and stated the vehicle's, "PCS and Lane Assist we still on." The owner stated she received knee and wrist injuries, and that the other party was also injured and "went to the hospital". Although it was stated that Toms River Police Department responded, the owner did not state the incident location or if the police created a report. On May 16, 2024, Toyota completed a vehicle inspection field report related to the crash event and obtained the PCS Freeze Frame Data (PCS FFD) and Event Data Recorder (EDR) indicating that ADAS L2 was engaged in the recorded moments prior to the crash. The PCS FFD recording contains approximately 8 seconds of pre-crash data and images. From the PCS FFD, Toyota observed the following: Dynamic Radar Cruise Control (DRCC) was engaged from the start of the recording through the crash event. Lane Trace Assist (LTA) was engaged from the start of the recording until it was cancelled at approximately 2.2 seconds prior to the crash event due to vehicle deceleration. The Pre-Collision System (PCS) Alarm (FCW) activated between 1.2 seconds ad 0.7 seconds prior to the crash event. The system did not provide PCS braking (AEB). Driver brake input did not occur prior to the crash event. |

What this table is and is not. Rows are the operators' own filings, not verified cases, and a filing does not establish fault. NHTSA redacts most Level 2 narratives as confidential business information, so the text column is empty for most Tesla rows; the structured columns are not redacted. The table joins NHTSA's current files (incidents from June 2025, after the third amended order) with its 2021-2025 archive, which used an older layout: severity labels, roadway and weather fields differ between the two sets, and the source_set column in the CSV says which one a row came from. Level 2 reports are shown here because the file contains them, but they are not cases in the register, which counts a vehicle as a robot only when nobody inside is responsible for driving. Source: NHTSA SGO 2021-01. The themed reading of the driverless narratives is on the reports page.
