NHTSA publishes the crash reports as three CSV files of 116 columns each, with several versions of the same report mixed in. This is the same data as one table: driverless systems (ADS) and Level 2 driver assistance side by side, 2021 to today including NHTSA's archive, one row per report at its latest version, every row traceable to its NHTSA report number. Free, CC BY 4.0, refreshed from NHTSA on every pipeline run.

While in autonomous mode, the Waymo AV began to enter the right lane of a protected double left turn lane ('Lane 2') from northbound [XXX] to westbound [XXX]. A pickup truck heading southbound on [XXX] had completely crossed the double-yellow lines of the northbound double left turn lanes and positioned itself in the left lane of the double turn lane ('Lane 1') facing the wrong direction against traffic and was preparing to make a left turn across Lane 2 and the other two northbound lanes into a shopping center parking lot on the east side of [XXX]. The Waymo AV was traveling in Lane 2 when the pickup truck began to initiate its left turn from Lane 1. The Waymo AV braked, and the trained human driver in the Waymo AV (in the driver's seating position), transitioned the vehicle from autonomous to manual mode. As the pickup truck made the left turn, the driver's side panels of the pickup truck made contact with the driver's side rear corner of the Waymo AV causing moderate damage to the Waymo AV. Damage to the pickup is currently unknown. At the time of contact, the Waymo AV was in manual mode. Waymo is reporting this crash under Request No. 1 of Standing General Order 2021-01 because the Waymo AV was towed from the scene. The vehicle was traveling South-bound on [XXX], an eight-lane divided road, operated by the vehicles Level 4 Automated Driving System and supervised by a safety driver in the drivers designated seating position, and with a safety operator in the front passenger seating position. Subject vehicle moved to the right of the lane to turn right onto West-bound [XXX], a two-lane road, and made contact with a curb at approximately 13-miles per hour. The safety driver was monitoring the behavior of a vehicle passing in the left lane for signs that it might enter subject vehicles path and did not see subject vehicle moving toward the curb in time to intervene and avoid contact. During the incident, the safety driver disengaged the Automated Driving System, and then pulled the vehicle to the side of [XXX] and inspected the vehicle and curb for damage. After the inspection, the safety driver drove the vehicle back to base with the Automated Driving System disengaged. Subject vehicle did not sustain tire or wheel damage, but the contact resulted in misalignment. No other agents were involved, no injuries were reported, and law enforcement was not called to the scene. Post-Crash Information: In addition to telematics and video, we also have photographs of the tires, curb, and alignment calibration information. |

The driver's son reported that the vehicle then accelerated uncontrollably for 260 yards and crashed into a tree where it came to a stop. Additional investigation shows that the subject vehicle was traveling at 77mph with some form of cruise control engaged (either basic or adaptive cruise control) in the right lane of a six-lane, divided highway. The driver was unbelted, with a chime defeat device latched into the buckle of the driver's seatbelt. The lane in which the subject vehicle was traveling terminated at an exit ramp. Warnings that the lane was going to end were provided as early as 2000 feet (over 600 yards) in advance, and signs were posted cautioning drivers to reduce speed to 20mph on the exit ramp. For unknown reasons, the driver did not follow the lane onto the exit ramp and instead continued traveling straight off the road. Ford understands that the vehicle struck the exit sign and then continued off-road and struck a stump and then a tree. The driver of the subject vehicle was fatally injured in the crash. Ford records show that the subject vehicle was not equipped with the operating system necessary to make BlueCruise functional. The subject vehicle was equipped with both ACC and LCA. Ford has confirmed that the subject vehicle was being operated with some form of cruise control engaged when the incident occurred. Ford has not been able to confirm whether LCA was engaged at the time of (or within 30 seconds before) the accident. |

What this table is and is not. Rows are the operators' own filings, not verified cases, and a filing does not establish fault. NHTSA redacts most Level 2 narratives as confidential business information, so the text column is empty for most Tesla rows; the structured columns are not redacted. The table joins NHTSA's current files (incidents from June 2025, after the third amended order) with its 2021-2025 archive, which used an older layout: severity labels, roadway and weather fields differ between the two sets, and the source_set column in the CSV says which one a row came from. Level 2 reports are shown here because the file contains them, but they are not cases in the register, which counts a vehicle as a robot only when nobody inside is responsible for driving.
