GM's current understanding of the crash is based on OnStar telematics data and the driver's statements after the crash to an OnStar representative. Before the crash, the vehicle was proceeding straight on a freeway road with Super Cruise engaged. The driver reported to OnStar that the cause of the crash was his inadvertent contact with the steering wheel that occurred when he bent down to pick something up off the floor of the vehicle. This is consistent with telematics data, which indicates that the driver provided a manual override input into the steering wheel approximately four seconds before the crash. This steering wheel input simultaneously disengaged Super Cruise and caused the sequence of events that led to the vehicle crashing into a fixed on-road barrier. Super Cruise was not active at the time of the crash. After the Crash, OnStar Emergency contacted the driver and found out the driver was alert and responsive, and requested emergency assistance. GM believes, but has not confirmed, that the vehicle was towed from the scene. Telematics data indicates that the driver was not belted at the time of the crash. GM does not know whether the driver or any vehicle passengers were injured during the crash. Since the time of submission, there is no new information at this time to update.

The 2021MY Kia K5 was traveling northbound on [XXX], in the inside lane approaching the Chevrolet Impala. A 2008MY Honda Civic was traveling northbound on [XXX] in the inside lane behind the Kia K5. Subsequently, the Kia K5's front struck the Chevrolet Impala's drivers side rear door and rear quarter panel. After impact, the Impala traveled in a northwesterly, counterclockwise direction into a concrete barrier wall and bounced off the barrier wall and redirected to a northeasterly counterclockwise direction across the northbound lanes when the Honda Civic's front collided into the Chevrolet Impala's and left rear door. The Chevrolet Impala came to rest facing north, blocking the inside lane. After impact, the Kia K5 traveled in a northeasterly direction across the northbound lanes and came to a final rest on the outside grass shoulder facing west. The Honda Civic came to final rest blocking the outside lane adjacent to the Chevrolet Impala. The subject Kia K5 is equipped with Highway Driving Assist (HDA). HDA is a supplemental system that helps maintain a set distance and speed from the vehicle ahead when driving on a highway and helps keep the vehicle between lanes while driving. HDA may not be able to recognize all traffic situations and may not detect possible collisions due to limitations of the system. Obstacles such as vehicles, motorcycles, bicycles, pedestrians, unspecified objects and structures that may collide with the vehicle may not be detected. The owner (father of the driver) of the Kia K5 contacted Kia's Consumer Affairs Department on August [XXX], 2021 indicating that his son was involved in an accident. The father was not a passenger or witness to the accident. The father reported that HDA was engaged at the time of the incident and stated that his son "had the autopilot on and there was a vehicle in the middle of the road and it [REDACTED, MAY CONTAIN CONFIDENTIAL BUSINESS INFORMATION] did not notify his son." According to the father, the police removed the "black box" from the vehicle. The claims made by the driver's father have not yet been confirmed, and we note that the subject vehicle has no "autopilot" nor does Kia America advertise any ADAS feature as "autopilot." Kia has no further information at this time.**9/[XXX]/2021 - No incremental information available as of 9/[XXX]/2021 2:26PM (PDT) |

The customer did indicate the Highway Drive Assist mode was on. The customer alleges that the vehicle left the lane of travel when driving a steep curved road, initiating a crash. Hyundai notes that the ODD of the Highway Drive Assist system is limited by certain environmental factors, such as weather and curvatures in the path of travel, requiring the driver to remain engaged at all times while operating the vehicle. Hyundai is attempting to gather additional details on the incident for a possible vehicle inspection. *10-Day Update* Hyundai is attempting to inspect the incident vehicle and download the EDR report for further analysis. *Monthly Update #1* Hyundai inspected the vehicle on October [XXX], 2021. The inspection and associated EDR report is currently under review. The results and assessment will be provided in the next monthly update and in the next monthly "deep dive" discussion with ODI. *Monthly Update #2 - Final assessment* Hyundai reviewed the traffic report and deduced that HDA could not have been engaged during the incident. HDA is only available for approved highways such as federal interstates. Hyundai also reviewed the EDR report and confirmed the driver entered the turn with Smart Cruise Control ("SCC") engaged at a set speed of 80 mph. The accelerator pedal was depressed throughout the event. The brake pedal was never depressed. Lane Following Assist ("LFA") engagement could not be confirmed however, a manual steering maneuver was performed approximately 2 seconds prior to the incident and the ESC engaged, indicating a vehicle slip. Hyundai deduced that the operator could have entered the turn at a high speed, realizing that the vehicle may not be able steer itself through the curve, and performed a significant steering maneuver in an attempt to center the vehicle within the lane, causing the vehicle to lose control. Hyundai notes that the HDA/LFA/SCC systems are not Level 2+ systems and are not designed nor marketed to provide complete "hands-free" driving to the operator. Drivers must be ready at all times with their hands on the wheel to insert themselves into a manual driving task. Hyundai has implemented driver attention warning systems that alert drivers up to 3 consecutive times if hands are not detected on the steering wheel for a brief period of time. |

The vehicle made an abrupt shift to the left causing the vehicle to hit several orange construction barrels in the area impacting the left side of the vehicle. The customer was not injured as a result of this incident. A subsequent inspection by Volvo revealed that the windshield was replaced on June [XXX], 2021 however the ASDM (module integral in the functionality of Pilot Assist) was not calibrated after the installation as recommended by Volvo. Additionally a Sun Pass toll transponder mount was found beneath the collision avoidance systems on the upper portion of the windscreen. Within the Owners Manual, Volvo strongly recommends not to mount any electronic device near that area as it may cause an adverse effect or impact functionality of the collision avoidance/Driver assistance system. As the ASDM was not calibrated after the windscreen replacement and evidence of use of an electronic transponder, Volvo considers this item as closed.

Data was downloaded from the vehicle at the time of the inspection. We are currently analyzing the data recovered. Monthly Update for Report:1422-3730-2: Pilot Assist limited the steering torque before the vehicle left the lane. This was because the lateral acceleration exceeded the limit. The vehicle could not steer enough to follow the lane with the available torque. Its important to understand that the functions are supplementary aids. They cannot manage all situations in all conditions. The driver is always responsible for ensuring that the vehicle is driven in a safe manner. The driver failed to take control of the vehicle when the vehicle Pilot Assist could not provide enough steering torque for the vehicle to stay within its lane.

The turn off lane is short and does not appear to have visible line markings except for the faded white triangle in front of the median, which ends approximately a foot before the median. The concrete median is gray in color and seems to blend with a color of the road/pavement and the sidewalk - essentially blending in with the surrounding roadway. As the employee turned into the lane, and attempted to get into the parking lot, the vehicle tried to maintain the same course of travel, thus coming into contact with an edge of the curb/median. The damage was to the wheel/rim area.

The claimant did not specify the date of the incident, but claimed that the vehicle, a 2024 Lincoln Navigator, was towed to [XXX] Lincoln on March [XXX], 2025. The claimant alleged that he was driving the vehicle in town, with BlueCruise activated, when his car suddenly accelerated forward. He claims he avoided colliding with the vehicle in front of him, hit a loading dock, and cracked the vehicle's radiator. The claimant clarified that he was not injured during the incident and there are no other reported injuries. During a prior call with the Customer Relationship Center on March 21, 2025, the claimant mentioned he had installed aftermarket stereo equipment and that some factory components were disconnected when the equipment was installed. Following a dealer inspection, a service technician confirmed that the aftermarket stereo equipment was causing numerous electrical issues and that factory components had been disconnected during its installation. Furthermore, the claimant admitted to previously hitting a pothole and a sapling tree, causing front-end damage. It remains unclear if this incident is related to the loading dock incident. On March [XXX], 2025, the customer reported issues with the headlights auto leveling to the ground, BlueCruise malfunctioning, and vehicle alignment. He attributed multiple accidents to these malfunctions. Ford has confirmed that the vehicle is equipped with Co-Pilot 360 Drive 2.0, which includes Adaptive Cruise Control (ACC) and Lane Center Assist (LCA) and BlueCruise technology. At this point, Ford has not been able to identify any related L2 connected vehicle data and is still investigation whether these features were active at the time of the incident. Ford has initiated communication with the claimant to gather more detail, including the location of the incident. BlueCruise can only be engaged and operated within the designed BlueCruise Zones. The feature cannot be operated for off-highway use. |
What this table is and is not. Rows are the operators' own filings, not verified cases, and a filing does not establish fault. NHTSA redacts most Level 2 narratives as confidential business information, so the text column is empty for most Tesla rows; the structured columns are not redacted. The table joins NHTSA's current files (incidents from June 2025, after the third amended order) with its 2021-2025 archive, which used an older layout: severity labels, roadway and weather fields differ between the two sets, and the source_set column in the CSV says which one a row came from. Level 2 reports are shown here because the file contains them, but they are not cases in the register, which counts a vehicle as a robot only when nobody inside is responsible for driving.
