On August [REDACTED, MAY CONTAIN CONFIDENTIAL BUSINESS INFORMATION] 2026, a Torc autonomous vehicle ("Torc AV") operating in the Fort Worth area was travelling eastbound on the single lane [REDACTED, MAY CONTAIN CONFIDENTIAL BUSINESS INFORMATION] Texas U-turn with the ADS engaged. The Torc AV stopped at the stop sign to take a left [REDACTED, MAY CONTAIN CONFIDENTIAL BUSINESS INFORMATION]. Once traffic cleared, the Torc AV began to proceed forward, taking a wide left turn to accommodate the standard 53-foot dry van trailer. During this turn, a gray Chevrolet Malibu behind the Torc AV attempted to illegally pass the Torc AV by undercutting it on the left by departing the marked travel lane into the striped shoulder. The Malibu slowed past the stop bar before the Torc AV trailer had completed the turn. As the trailer turned, the driver's side skirt of the trailer impacted the passenger side of the Malibu, followed shortly by the tandem wheels.
The ADS remained engaged through initial contact. The safety driver quickly disengaged ADS [REDACTED, MAY CONTAIN CONFIDENTIAL BUSINESS INFORMATION] after the tandem wheels contacted the Malibu, assumed manual control, and then pulled the truck to a stop in the right-hand lane [REDACTED, MAY CONTAIN CONFIDENTIAL BUSINESS INFORMATION]. Shortly thereafter, the Malibu completed its left turn and parked behind the Torc AV. The Torc trailer had superficial damage to the side skirt, and the Malibu suffered body damage to the front and rear passenger side doors. No airbag deployment occurred in either vehicle. Both vehicles were driven from the point of impact to the roadside under their own power.
Fort Worth Police arrived and released the Torc AV from the scene. No injuries were reported at the scene.

The subject vehicle was traveling at 64mph in the center eastbound lane on [XXX] near [XXX], having recently passed a pickup truck (vehicle #2) towing a trailer in the right lane. A large piece of road debris in the right lane caused vehicle #2 to slow and veer onto the right shoulder. A second piece of large road debris was observed by the subject vehicle's operator ("VO") in the center lane approximately 80 meters ahead of the subject vehicle.
The VO took control of the truck and began to steer the subject vehicle to the right, but upon seeing vehicle #2 accelerating and re-entering the right lane from the shoulder, the VO instead steered the subject vehicle into the left lane. After passing the debris, the subject vehicle returned to the center lane and continued driving eastbound on the highway.
A post-event review of the subject vehicle's cameras showed an ambulance with its emergency lights off (vehicle #3) was traveling in the left lane to the rear of the subject vehicle as the subject vehicle began entering the left lane. When the subject vehicle entered the left lane, vehicle #3 maneuvered onto the left shoulder and came to a stop alongside the median.
The subject vehicle made no contact with vehicle #2 or vehicle #3. PlusAI has not been contacted by any road user with regard to this event.

The Zoox vehicle braked hard and attempted to steer left, but contact was unavoidable. The right side front fender, wheels, bodyside, and door panels made contact with the outside edge of the opening door of Vehicle 2. The Zoox vehicle came to an immediate stop and was later towed from the scene to be inspected for damage. First responders arrived at the scene. The passenger of the Zoox vehicle did not report any injuries. Later media stories reported the driver of Vehicle 2 had a "smashed hand" and his "hand was hurting, and that he was also experiencing pain to his neck area and lower back" based on a third party's account, and that the driver "declined their offer to take him to the hospital."
Zoox vehicle camera footage shows clearly that no part of the robotaxi came into contact with the driver themselves, and that their hands, leg, and body did not interact with either the robotaxi or the parked vehicle's door at any moment during the contact event.

On October [XXX], 2025, at approximately 2:30 PM PT, a Motional AV with VIN [XXX] was operating in autonomous mode in Las Vegas, Nevada, with a safety driver in the drivers seat. The vehicle was traveling northbound on [XXX] in the second of four lanes from the left, approaching the intersection with [XXX].
While stopped at a red traffic signal, an adult male pedestrian began crossing [XXX] from the west side of the street, several hundred feet away from a designated crosswalk. The pedestrian proceeded southbound across multiple lanes and paused on a median located ahead and to the left of the AV. The AV detected and classified the pedestrian the entire time.
When the traffic signal turned green, the pedestrian remained stopped on the median. Given that the pedestrian remained stopped in place, the AV began to move forward slowly at approximately 2.5 mph and continued to correctly detect and classify the pedestrian. The pedestrian, holding what appears to be an item in a plastic bag, suddenly stepped into the far-left turn lane ahead of the AV, the AV immediately came to a complete stop still in the second lane. The pedestrian continued crossing, making a light physical contact with the rear driverâs side of the vehicle before continuing across the remaining three lanes of traffic still outside of the crosswalk.
The vehicle operator then disengaged the autonomous system and pulled over to complete an incident report. No police were notified. There were no injuries, no damage, and no airbag deployments. The pedestrian did not stop at the scene and the identity of the pedestrian is unknown.

The May vehicle was travelling in an autonomous vehicle (AV) only lane approaching an intersection. A vehicle travelling to the left of the May shuttle (ego) in the non-AV lane performed a right hand turn across the May vehicle's path. Given the full stopping power and the limited space with which the ego had to stop, the ego was physically unable to stop in time before impact. The primary factor that led to this collision was human error on the part of the other vehicle that was required to yield to through traffic in the AV lane but did not. Right turns across a straight lane are uncommon, and drivers are typically not used to them, however there are several signs that instruct other traffic to yield to the vehicles in the AV lane. There were no autonomy faults or errors which led to this incident.
Update:
There was no indication of injury or request for medical attention at the scene. The police report states "Driver 1 complained of neck pain." For this reason, we are updating the highest severity injury alleged field to reflect the "minor w/o hospitalization" injury level. The remaining content of this report is unchanged from the initial report submitted on September [XXX], 2025, Report ID #[XXX].

On June [XXX], 2025, a Beep-operated Ford E-Transit (004D) equipped with an automated driving system developed by Oxa Autonomy Ltd. was involved in a collision with a third-party vehicle in Jacksonville, FL.
While navigating northbound on [XXX], vehicle 004D approached the intersection of [XXX] under automated control. As vehicle 004D proceeded through the intersection, a 2020 Nissan Pathfinder navigating eastbound on [XXX] entered the intersection and struck vehicle 004D on the drivers door. Vehicle 004D had right of way through the intersection with no stop or yield the other vehicle had been stopped at a stop sign prior to the collision. There were no injuries reported at the scene, and both vehicles were driven away from the site of the collision.
Prior to the collision, the safety operator in vehicle 004D stated that they noticed the other vehicle entering the intersection, applied the vehicles brakes, and steered away from the path of the other vehicle. Though they were able to bring vehicle 004D to a complete stop, they were unable to completely avoid the collision. Review of dashcam footage and of CCTV footage covering the intersection corroborate this narrative. The safety operator was referred for a post-accident drug and alcohol testing after the incident. They tested negative for any impairment.
Analysis of the ADS logs determined that the ADS had detected the potential collision and had planned an emergency braking action. The safety operators intervention revoked control of the vehicle from the ADS preventing any automated braking actions.
The [XXX] responded to the accident scene. The driver of the other vehicle was cited for failing to stop at the stop sign, and the JSO CSO noted that vehicle 004D was not at fault for the accident.
